Under the direct supervision of a team leader (Registered Nurse or Licensed Practical Nurse), the nursing assistant provides for the physical, psychological, and emotional needs of Health Center residents, with emphasis on preservation of dignity and confidentiality. He/she is expected to be courteous and helpful to all residents, families, visitors, and co-workers. This individual must practice according to state and federal regulations and established policies and procedures of the facility. 

Responsibilities:

  • Observes and responds promptly to the individual needs of the resident with courtesy and respect in a dignified manner 

  • Provides personal care to residents including bathing, dressing, grooming, feeding, toileting, transferring, ambulation, range of motion, etc. in accordance with the resident's plan of care 

  • Monitors vital sign: temperature, pulse, respiration, blood pressure, and intake and output 

  • Reports abnormalities promptly to Charge Nurse 

  • Reports all pertinent information to the Charge Nurse regarding resident's physical and psychological conditions (including but not limited to: appetite, ability to swallow, change in ability to transfer or ambulate, changing safety awareness, toilet habits, physical appearance, complaints of pain, altered skin integrity, and mood changes) 

  • Maintains a clean and safe environment for residents and staff 

  • Keeps all areas clean and orderly 

  • Ensures cleanliness of equipment 

  • Provides for tidiness or orderliness of resident's drawers and clothes closet.

Qualifications

  • High school diploma or GED preferred 

  • One year long term care experience preferred 

  • Demonstrate an appreciation and a genuine desire to serve the elderly 

  • Understand and function within all Sherwood Oaks Policies and Procedures 


Licensure/Certifications: 


  • Current PA Certified Nursing Assistant (CNA) certification required 

  • Cardio Pulmonary Resuscitation (CPR) certification is also required within 90 days of employment 

  • Act 34 and Older Adult Protective Services Act clearance (when required) as needed

Salary Range: $11.80 to $11.80
