Description

Purpose:
To assess and evaluate patients that are referred UPMC VNA from Skilled Nursing Facilities, Assisted Living Facilities and Personal Care Homes for appropriateness and a smooth care transition. To promote the continuum of post acute services provided by UPMC VNA. The Community Clinical Liaison promotes the achievement of home health admission requirements through direct contacts and appropriate referrals sources.

The Community Nurse Liaison will work Monday through Friday from 8:00am-4:30pm. This position will support the Monroeville area.

Responsibilities:
  • -Analyzes and solves customer concerns by preparing and providing solutions to problems and follows up on referrals to ensure customer satisfaction and promote positive outcomes
  • -Applies nursing skills to promote and ensure customer satisfaction
  • -Assist in planning patients home care needs
  • -Complete appropriate documentation and paperwork accurately and within acceptable time frame
  • -Conduct on site assessments and establish priorities in the allotted time frame to UPMC VNA, maintaining accuracy, attention to detail while maintaining confidentiality and privacy in compliance with HIPPA.
  • -Develop and maintain knowledge of the UPMC VNA services and programs
  • -Develops and maintains a thorough understanding of the state of the art nursing stands of practice
  • -Identifying and recommending services to assure safe continuum of care
  • -Maintains high moral and business ethics in carrying out responsibilities and duties
  • -Makes sales and admission development contacts while in facilities and with other referral sources
  • -Provide informative literature about UPMC VNA to referral sources.
  • -Provide resources and education services about UPMC VNA services and admission standards to patient, families, social workers, care managers and physicians
  • -Provides management with full details of process improvement needs on nursing assessment and patient care issues
  • -Provides management with performance improvement information such as productivity and referral contacts
  • -Provides management with relevant information and offers input regarding external developments and opportunities
  • -Review patient records and obtain any relevant patient information assessing for patient appropriateness and medical stability to facilitate a timely admission and transfer to patients home
  • -Works with facility personnel to handle customer concerns and patient care issues
  • Performs all duties and responsibilities according to the philosophy and standards of UPMC Health System and UPMC VNA embracing the system wide core competencies which include: A) Making quality patient care and patient satisfaction the number one priority B) Initialing and participating in performance improvement initiatives. C) Conveying courtesy, respect enthusiasm and positive attitudes with patients, staff, medical staff and visitors. D) Maintaining confidentiality at all times.

Qualifications

  • BSN and/or BS in health related field preferred
  • At least 2 years experience in home health or a skilled facility required 
  • Experience in marketing preferred
  • Must have ability to work independently, multitask and problem solve 
  • Demonstrates professional oral and written communication skills
  • Demonstrates the ability to establish and maintain effective working relationships with variety of internal and external referral sources

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

Salary Range: $23.56 to $40.77
