Description

Are you a Hospice Nurse looking to gain leadership experience and take your career to the next level? This is the role for you! The Patient Care Supervisor will Manage and implement clinical services for the Hospice program. This will be done in a manner consistent with efficiency, cost-effectiveness and regulatory compliance.

Responsibilities:
  • Assist Hospice Medical Director with physician-to-physician contacts and staff communication with Medical Director.
  • Assure Hospice trained and competent staff are available to provide care. Interview and recommend staff for hire based on program needs and candidate qualifications. Participate and coordinate orientation to Agency, Hospice program and interdisciplinary team. Supervise and conduct regular performance reviews. Evaluate and make recommendations regarding employee status, assignments, disciplinary action and termination of staff. Assure staff have obtained required competency for their job classification on hire and ongoing. Assure inservice/continuing education opportunities are available to keep staff skills upgraded. Develop and implement strategies to recognize stressors experienced by staff in working with Hospice patients, families and caregivers. Collaborate with management team to access staff support.
  • Assure Hospice trained and competent volunteers are available to provide home care, bereavement and clerical support.
  • Demonstrate effective leadership in working relationships with Hospice staff, agency personnel and community leaders.
  • Demonstrate support of Agency mission and philosophy in delivery of services.
  • Direct and manage the clinical services for the Hospice Program in assigned geographic area. Assure timely assessment and admission of patients into Hospice Program based on Hospice admission criteria. Implement Hospice clinical and operational policies consistently throughout service area. Assure plan of care is developed and implemented by Hospice interdisciplinary team. Assure care is coordinated in all treatment settings to include homes, nursing homes and inpatient settings. Assure revocated patients meet criteria to revocation and receive necessary information about community resources for continued care. Monitor utilization patterns to assure patient receives required amount of services while avoiding over and under utilization of services.
  • Endeavor to achieve and maintain quality, employee/customer satisfaction in the delivery of services to internal/external customers.
  • Evaluate own needs for support and use identified system to meet the need.
  • Implement strategies to assure patient, family and caregiver, referral source and physician satisfaction with services.
  • Maintain and protect patient, client and Agency confidentiality.
  • Maintain and support Hospice interdisciplinary team. Assure core disciplines are represented at all meetings. Facilitate communication links among disciplines involved in developing and implementing plan of care.
  • Participate in Hospice Program and Agency-wide performance improvement activities. Assist staff to identify and resolve problematic internal/external processes that impact patient care and operations. Respond to and resolve internal and external customer concerns in a timely manner. Implement infection control program. Address incidents and provide follow up/counseling to staff, as required. Implement patient/family complaint process and assure timely resolution of concerns.
  • Participate in inservice programs, orientation, quality assurance, utilization review and performance improvement activities.
  • Promote and enhance relationships with physicians and health care community.
  • Promote personal and co-worker safety during work duties.
  • Provide administrative oversight of the following functions: Assure only contracted vendors are utilized for durable medical equipment, pharmacy, parenteral therapy, laboratory, inpatient acute and respite care and transportation services. Provide inservice, as appropriate, to facility staff in skilled nursing facilities, inpatient and respite sites. Negotiate services with third party payers for individual patients. Conduct record review as required by third party payers and regulatory bodies to assure accuracy of assessment and clinical documentation. Assure clinical record documentation includes information required to avoid medical and technical denials. Assure program is in compliance with accreditation requirements, third party payer regulations, federal and state laws. Participate in development of policies, procedures, clinical protocols and forms. Participate in development of budget. Manage and deliver care in cost-effective manner, which demonstrates an understanding of Hospice reimbursement methodologies. Assure staff meet productivity standards through monitoring of caseload and visits per day, month or quarter. Prepare management reports on a regular and timely basis.
  • Provide direct patient care as outlined in the Hospice RN job description.
  • Represent and participate in Agency professional and community activities as requested.
  • Seek opportunities for professional growth to enhance relevant job-related skills.
  • Share responsibility for after-hours call with colleagues.

Qualifications

  • Graduate of accredited school of nursing, BSN preferred;
  • minimum three years clinical experience in nursing and one year in home care/Hospice preferred.
  • Competency in computer skills in a Windows-based environment preferred.
  • Access to transportation in the Agency service area with valid Pennsylvania drivers license and auto liability insurance.

    Licensure, Certifications, and Clearances:
    CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire. Registered, certified or licensed as required by state law for the profession.
