Description

Purpose:
Provide designated care under the direction of the primary service provider in the home in order to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.

Are you interested in growing your career in Healthcare?  This is a part-time position which is responsible for providing care in the Huntingdon County area.  Mileage is reimbursed at 54 cents/mile.  

Responsibilities:
  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
  • Communicate appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Maintains and completes documentation accurately and promptly while in the patient�s home and within the agency and regulatory standards.
  • Participate in all required agency meetings.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Proven ability to facilitate change.
  • Provide safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.

Qualifications

High School Diploma or GED - Required Must have successfully completed a training and competency evaluation program as specified in 484.80 (Home Health) and/or 418.76 (Hospice) as applicable: Condition of participation: Home Health aide services. One (1) year experience - Preferred; Active employment within the past 24 months as a Certified Nurse Aide in good standing.

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Act 73 FBI Clearance
  • Automotive Insurance
  • Cardio Pulmonary Resuscitation
  • Driver's License
