Purpose:

The pediatric hematopoietic cell transplant clinical pharmacist is an integral member of the bone marrow transplant team who provides a variety of pharmacy services to the healthcare team, the patient, and caregivers in an effort to optimize collaborative patient-centered care focused on patient safety. Provide pharmaceutical care, medical usage evaluations, adverse drug reaction reporting, therapeutic drug monitoring and clinical interventions.

Responsibilities:
  • Chemotherapy and medication counseling.
  • A member of the Therapeutic Drug Monitoring Service and 6th floor clinical pharmacist liaison. 
  • Assist in ensuring departmental compliance with regulatory and accrediting agency standards, guidelines and quality assurance standards.
  • Assist in pre-accepting University of Pittsburgh School of Pharmacy students.
  • Assume responsibilities, tasks and staffing duties as a dispensing Pharmacist as deemed necessary by the Director of Pharmacy. This may include weekends, and the staffing of any assigned shift.
  • Collaborate in the coordination of the staff's clinical development. Provide continuing education materials and lectures and provides input to the performance evaluation of other clinical staff members.
  • Edit and update the Pediatric Drug Therapy Handbook and Formulary.
  • Ensure patient safety through ensuring the monitoring of adverse drug effects via patient chart review; notify physicians of occurrences; make recommendations to physicians for drug dosing in patients with compromised renal and/or liver function.
  • Participate in the Adverse Drug Reaction Program including evaluating and reporting adverse drug reactions.
  • Participate in the Medication Usage Evaluation Program including planning, research, design, and evaluation and reporting results.
  • Participate in the department's Quality Improvement Program.
  • Perform cost reduction measures.
  • Provide drug information to various patient care departments as necessary.
  • Provide pharmaceutical care and monitor drug therapy to affect positive patient outcomes.
  • Provide professional education to medical, nursing and pharmacy staff members and students.

Qualifications

  • Bachelor's degree or Doctor of Pharmacy (Pharm D) is required.
  • Two years of hospital pharmacy experience is required.  
  • Hematopoietic cell transplant (HCT) experience is preferred.
  • Pediatric oncology pharmacy experience is preferred.
  • Post graduate pediatric residency/pediatric oncology pharmacy residency is preferred. 

Licensure, Certifications, and Clearances:
  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Act 73 FBI Clearance
  • Pharmacist license
  • Board Certified Pediatric Pharmacotherapy Specialist preferred
  • Board Certified Oncology Pharmacotherapy Specialist preferred
