Description

  • Supervises frontline staff in Home Health, assuming responsibility for their work performance and providing oversight of work flow and processes. 
  • Responsible for all clinical activities of assigned nursing and other professional staff. 
  • Oversees all case management activities and meets or exceeds established outcome measures.

Responsibilities:
  • Complete field visits at least monthly to assess care, service and patient satisfaction.
  • Coordinate education for staff, patients and families.
  • Ensure all productivity and documentation standards are met.
  • Establish evidence-based protocols to guide effective practice.
  • Evaluate patient care and outcomes through effective utilization of reports.
  • Provide input into department's budget; supports the financial goals of UPMC Susquehanna through individual actions and management of resources.
  • Review supply chain management protocols daily.
  • Rewards, disciplines and evaluates direct reports, using independent judgment; provides input into hiring and involuntary termination decisions.
  • Supervises frontline staff in Home Health, assuming responsibility for their work performance and providing oversight of work flow and processes. Responsible for all clinical activities of assigned nursing and other professional staff.
  • Work directly with staff on patient experience, quality and cost measures to meet or exceed established targets.

Qualifications

  • Two years of home health experience or two years of supervisory experience in a healthcare setting required. 
  • Physical Therapist licensure in the Commonwealth of Pennsylvania required.

Licensure, Certifications, and Clearances:
  • Physical Therapist licensure in the Commonwealth of Pennsylvania
  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Act 73 FBI Clearance
  • Basic Life Support
  • Physical Therapist
