Susquenanna is currently seeking a Registered Nurse to function as a Team
Leader for the Home Health office at Divine Providence! The Team Leader will
primarily be responsible for providing clinical support in the Home Health
office as well as assisting with home visits as needed. This is an excellent
opportunity for a Registered Nurse to take initiate to function as a leadership
resource for a team of dedicated home health nurses!
Responsibilities:
Attend compliance training and
adhere to the organization standards of conduct, policies and procedures
Collaborates with Clinical
Supervisor on employee performance reviews. Makes supervisory visits with
staff to ensure the delivery of quality patient care.
Continually develops
professional skills by assuming responsibility for ongoing education.
Facilitates accurate
documentation and ensures optimal patient care and utilization of services
through system of critical pathways to ensure that outcomes are achieved
while containing costs per episode.
Facilitates communication with
inter-intradisciplinary team members, physicians, and other professionals
and by fostering and promoting a positive work environment
Focuses on customer service and
continually strives to perform the duties of their job in a manner that
will result in optimal patient satisfaction
Identifies, develops and
participates in process improvement opportunities within the home health
agency that will enhance the quality of services we provide
Oversees activities of the team
including assignment of new patients to multidisciplinary staff.
Coordination of interdisciplinary services to patients, coordination and
scheduling of visits to ensure that productivity is achieved. Ensures that
orders for services and treatment are obtained by the Case Manager prior
to delivery of care. Provides discipline specific direct patient care as
scheduling requires.
Performs duties and job
responsibilities in a fashion which coincides with the service management
philosophy of the organization towards patients, visitors, staff, peers,
physicians and other departments within the organization.
Proven ability to facilitate
change
Qualifications
Baccalaureate degree-Preferred
Minimum of two (2) years
healthcare experience - Required
Minimum of one (1) year home
health experience - Required
Minimum of one (1) year management
experience - Preferred
Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include
both a didactic and skills demonstration component within 30 days of hire
Act 33 Child Clearance with
Renewal
Act 34 Criminal Clearance with
Renewal
Act 73 FBI Clearance
Automotive Insurance
Basic Life Support
Cardio Pulmonary Resuscitation
Driver's License
