Purpose:

Designs and analyzes reports highlighting opportunities to improve clinical performance/patient care. Assist the teams (department and facility) in the proper analysis and presentation of information using various clinical and administrative systems and the corporate database. Reviews medical records for the determination of data collection as related to analysis of documentation, process issues, demographic elements, and as part of understanding and proceeding with database design / re-design, analyses, and improvement process. Needs to understand the clinical needs of the committee/projects to further progress the process improvement using analyses and recommendations. Manages designated external reports, including analysis, processing, submission, and final review.

Responsibilities:
  • Care Delivery: Maintains patient confidentiality and ensures data accuracy. Understand the clinical/technical needs of the program/project to help progress the process improvement initiatives. Track department and related facility quality projects.
  • Collaborative Practice: Work well with the CQII department, facility committees, project leads and physicians. Provide consultation and assistance for all approved data requests from the applicable registry data registry including requests on abstract and risk adjustment methodology. Communicate applicable registry updates to project team and team leads in a timely manner. Identify current /future data challenges, develop meaningful recommendations and provide timely updates on issues encountered.
  • Leadership: Exhibit advanced problem solving using various information sources. Must be able to determine what the best source of information is and develop documentation on how to standardize abstraction procedures. Ability to work independently with minimal supervision. Develop an in-depth understanding of pertinent clinical information to be extracted from the medical record (concurrent and retrospective) and/or associated electronic patient information (MARs, AccessAnyware, Power Chart, Cerner, Medipac, etc) for inclusion in and submission to internal corporate databases and national data registries. Coordinate preparation, submission, and review of external reports and data submissions. Meet program/project deadlines and complete tasks within the amount of time prescribed by supervisor. Maintain high level of accuracy and timeliness.
  • Professional Growth: Stay abreast on all applicable registry related functions by participating in webinars / conference calls and regularly reading all applicable registry news releases, publications and associated documents. Master new computer software programs and upgrades, as applicable. Assist in organizing and leading discussions with multi-disciplinary teams, including but not limited to: senior management, department/division chiefs, chairs, physicians, etc. Prepare concise, meaningful analyses, narratives and recommendations to meet established goals and objectives.

Qualifications

  • Bachelors Degree in a healthcare related field with 2 years of experience in clinical data abstraction, clinical chart review, data entry or data analysis OR 
  • High School Diploma with 5 years of experience in clinical chart review or clinical data abstraction 2 years of experience in a healthcare facility (hospital surgery department, surgery clinic, clinical research or medical records) 
  • Previous experience as a Perfusionist, Abstractor, or Coder preferred.
  • Where applicable, complete all training modules of the ACS NSQIP and sit for the certification exam within 6-months of date of hire. 
  • Employees not passing the certification exam will be required to sit for and successfully pass the next available exam. 
  • Proficient in MS Office Strong written and oral communication skills

  • Act 34 Criminal Clearance
