Description

Purpose:
Ensures implementation of and provides overall direction for the organization's disease state management programs that utilize consistent protocols and scientifically proven interventions.

Responsibilities:
  • Assists with the development of patient education material within the scope of the disease state management programs.
  • Attends compliance training and adhere to the organization standards of conduct, policies and procedures.
  • Collaborates in the development of broad program goals and objectives and assumes accountability for their achievement.
  • Contributes to the development and oversight of a statistically sound method for gathering, aggregating, analyzing and reporting outcomes.
  • Contributes to the development of specific disease management program to enhance patient compliance, improve patient outcomes and provide fiscally sound services. Provides ongoing oversight to the provision of the services as set forth in the programs.
  • Focuses on customer service and continuously strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Identifies technological opportunities to maximize patient outcomes and staff efficiencies. Provides oversight to the organization's Telehealth program which includes installation/tear-down, monitoring and follow up findings when necessary/.
  • Identifies, develops and participates in process improvement opportunities within the organization that will enhance the quality of the services we provide.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Proven ability to facilitate change.
  • Serves as a liaison and consultant between referral sources, nurse liaisons, discharge planners, and the organization to ensure quality of care.
  • Serves as a member on agency committees.
  • Serves as a mentor to staff within the disease state management programs to ensure competence and consistency within the provision of services to attain positive patient outcomes.

Qualifications

  • Graduate of an accredited school of professional nursing - Required
  • Minimum of two (2) years healthcare experience - Required
  • Minimum of one (1) year home health experience - Preferred
  • Minimum of one (1) year management experience - Preferred
  • BSN/Baccalaureate Degree - Preferred
  • Master's Degree - Preferred
  • Specialty Certification, when applicable - Preferred
  • Cardiac Care Experience - Preferred
  • Utilization management/case management experience - Preferred
  • Basic computer skills - Preferred

Licensure, Certifications, and Clearances:
Specialty Certification, when applicable, preferred. CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Act 33 Child Clearance
  • Act 34 Criminal Clearance
  • Automotive Insurance
  • Basic Life Support or Cardio Pulmonary Resuscitation
  • Driver's License
  • Older Adult Protective Services Act
  • Registered Nurse
