Description

Purpose:
Supervises, guides and monitors activities of the multidisciplinary team to effect quality patient care through accurate documentation, ongoing education and coordination of the team's operations.

Responsibilities:
  • Attend compliance training and adhere to the organization standards of conduct, policies and procedures.
  • Collaborate with Clinical Supervisor on employee performance reviews.
  • Continually develop professional skills by assuming responsibility for ongoing education.
  • Ensure that orders for services and treatment are obtained by the Case Manager prior to delivery of care.
  • Facilitate accurate documentation and ensure optimal patient care and utilization of services through system of critical pathways to ensure that outcomes are achieved while containing costs per episode.
  • Facilitate communication with inter- or intradisciplinary team members, physicians, and other professionals and by fostering and promoting a positive work environment.
  • Focus on customer service and continually strive to perform the duties of job in a manner that results in optimal patient satisfaction.
  • Identify, develop and participate in process improvement opportunities within the home health agency that enhance the quality of services provided.
  • Make supervisory visits with staff to ensure the delivery of quality patient care.
  • Oversees activities of the team including assignment of new patients to multidisciplinary staff.
  • Coordination of interdisciplinary services to patients, coordination and scheduling of visits to ensure that productivity is achieved
  • Ensures that orders for services and treatment are obtained by the Case Manager prior to delivery of care.
  • Provides discipline specific direct patient care as scheduling requires..
  • Provide assistance with orientation, continuing education, performance and competency evaluation of personnel activities.
  • Provide direct patient care as scheduling requires.
  • Serve as liaison and consult between referral source, nurse liaison and infusion company.

Qualifications

  • Baccalaureate degree - Preferred
  • Minimum of two (2) years healthcare experience - Required
  • Minimum of one (1) year home health experience - Required Minimum of one (1) year management experience - Preferred

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Act 33 Child Clearance
  • Act 34 Criminal Clearance
  • Automotive Insurance
  • Cardio Pulmonary Resuscitation
  • Driver's License
  • Older Adult Protective Services Act
  • Registered Nurse
