Description

Purpose:
The primary function of the Service Coordinator is to provide professional assessment, service planning, service coordination, referral, re-evaluation services, crisis support and advocacy required for a safe and healthy community life which is manifested by stability in relationships, housing and employment. This is done in accordance with emerging mental health recovery principles.

Responsibilities:
  • Actively participate as part of the treatment team and assume a leadership role in assuring coordination and continuity of care.
  • Advocate for consumer and/or family choice of services and active participation in his/her recovery process through education and training
  • Assist the person served in developing and using natural supports
  • Assure there is cross system coordination of services and that services are being provided
  • Assure there is periodic assessment and cross system planning to meet the needs while utilizing strengths
  • Attend Agency, County and State training as appropriate and/or required.
  • Be a persistent advocate for those served and give feedback on system problems. Advocacy includes providing information, removing barriers, creating options and resolving problems.
  • Be the go-to resource for the person served and their family. Assure that effective safety net resources are in place
  • Clearly communicate what they can expect from the system and what the system will expect from them
  • Complete and document assessments, service plans, progress notes and other required documentation in timely manner.
  • Complete and implement strengthsbased service plans that are based on Recovery Principles and identify strategies for service coordination involvement in the recovery process.
  • Develop relationships that endure with persistent outreach even when there is reluctance to receive services
  • Maintain minimum required face-to-face contacts with consumers as per Pennsylvania Bulletin. Adjust frequency of contact as clinically appropriate.
  • Participate in 24 hour on-call rotation. Provide phone and face-to-face contacts after business hours. Is on site for all in-patient MH commitments. Ensures that all alternatives to hospitalization have been considered. Coordinates intervention when hospitalization is required.
  • Prepare for, convene and facilitate service planning meetings and provide follow up after meetings.
  • Provide a consistent positive outlook which encourages recovery and full inclusion in the community
  • Provide advocacy for consumers involved in 302 and other involuntary proceedings such as guardianship and custody proceedings.
  • Provide comprehensive assessment of consumer strengths and needs in all life domains.
  • Provide crisis support including assessment for diversion from the most restrictive levels of care.
  • Utilize Recovery Principles in efforts to promote consumer self-management of illness

Qualifications

  • Bachelors Degree in a Human Services field such as Social Work, Psychology or Counseling required. 
  • Other fields of study may be considered with coursework of at least 12 credits in Human Services. 
  • One year related experience preferred Prior experience working in community settings preferred. Safe driving record required.

Licensure, Certifications, and Clearances:
Complete the CANS/ANSA Certification within 6 weeks of employment.

  • Act 34 Criminal Clearance
  • Cardio Pulmonary Resuscitation
  • Driver's License
