Purpose:
The Community Peer Advocate position will help develop and track a new model of delivery of care. The candidate must provide non-judgmental support to patients and their families and establish and maintain positive, caring relationships with families in the Clinic in a culturally appropriate and sensitive manner and act as their advocate. The candidate must have good listening skills and ability to work with the care team to help ensure patients' active involvement in keeping their appointments for medical care.

Responsibilities:
  • Ability to establish and maintain positive, caring relationships with families in the Clinic in a culturally appropriate and sensitive manner and act as their advocate
  • Actively completes the necessary preparatory work prior to the appointment to ensure the patient keeps the appointment
  • Actively participates in Clinic quality improvement efforts and identifies opportunity for quality improvement to team membersTakes responsibility in improving patient satisfaction with the quality of care and service
  • Actively promotes strategies that help pediatric patients comply with follow-up care, including mental health services Serves as an active resource linking families with additional resources and services that support the entire family
  • Acts as a navigator, helping parents apply for public health insurance
  • Aids in the formulation of goals and plan of care for patients that involves the patient/parent as partner and considers the individual needs of the patient in a holistic manner, triaging patient situations and coordinating care with the team
  • Assists the team with all aspects of patient care including assessment, evaluation and education
  • Develops and maintains a specific registry to track show-rate, follow-up care, screening labs and questionnaires, participation in social services, mental health services, community engagement activities, wellness programs, and shared decision-making activities
  • Is available to families by phone to troubleshoot barriers to keeping appointments
  • Is knowledgeable in the principles of shared decision-making and completes an on-line course on this Actively promotes shared decision-making with parents and patients, participates in shared decision-making activities
  • Participates in community engagement activities
  • Reports regularly to the lead physician and the Family Support Center director
  • Supervision of patient scheduling, registration, medical records, and some data entry and processing
  • Works to help patients during transitions in care (such as during a hospitalization, subspecialty care, etc.)

Qualifications

  • High School Diploma or GED required.
  • College degree or Associate's degree with experience in program development, patient education, and/or social services preferred but not required.
  • Excellent interpersonal, written and verbal communication skills are required Medical terminology and third party health care coverage experience preferred.

 

Licensure, Certifications, and Clearances:

  • Valid PA motor vehicle license, access to a personal vehicle
  • Act 31 Clearance
  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Act 73 FBI Clearance
