Description

Purpose:
Manages and coordinates the Quality Assurance and Performance Improvement (QAPI) program and Infection Prevention Program for the assigned business unit. Serves as the primary consultant to clinical management for the QAPI program. Ensures that the QAPI plan is data-driven and focused on efforts that relate to regulatory requirements, quality indicators, healthcare best practices, and high volume, high risk and problem prone services provided by the organization. Directs and supports quality improvement teams through the process of data analysis, prioritization, plan development and plan implementation. Assists in the implementation of processes to support sustainability of implemented plans to ensure the highest quality of customer care and services.

Responsibilities:
  • Able to identify and obtain data from a multitude of sources and ensures accuracy of data/information obtained.
  • Able to support multiple performance improvement teams through the collection, prioritization, implementation and monitoring processes.
  • Assists in the design and implementation of a quality assurance performance improvement process that is based on regulations, best practices, effective / efficient processes, and identified needs of the organization for the assigned business unit.
  • Assists in the development and implementation of training programs, tools, and other documents to help staff successfully implement and maintain clinical initiatives or programs.
  • Assists teams to develop processes to ensure sustainability of new or changed processes.
  • Assists with the development of an annual infection prevention risk assessment, goals and surveillance plan for assigned branch offices.
  • Assists with the preparation of performance improvement documents.
  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
  • Collaborates with managers, clinical staff, and support staff from within and outside the department / organization as needed and related to improving outcomes.
  • Coordinates influenza, PPD, N-95 and other initiatives as related to infection control and prevention for the assigned branch offices.
  • Coordinates scheduling of work teams to ensure progression of performance improvement projects.
  • Ensures that performance improvement projects are data-driven and results focused.
  • Ensures that the branch offices are kept abreast of infection control issues as they develop and disseminates education in a timely fashion.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Incorporates performance improvement best practices and tools into the quality assurance and performance improvement framework of the organization.
  • Knowledgeable to current regulatory, accreditation and professional agencies (APIC) with regard to Infection Control and Prevention.
  • Knowledgeable to the appropriate performance improvement tools and techniques to meet the targets of a project.
  • Serves as a resource to all assigned business unit staff regarding infection prevention and control as it relates to patients and staff.
  • Serves as the primary consultant to clinical management for the QAPI program.
  • Systematically utilizes data to assist the organization to prioritize performance improvement projects in order to meet the mission, vision and goals of the organization.

Qualifications

  • Bachelor's degree in nursing or related field - Required
  • A minimum of five years of experience in health care - Required
  • Home Health experience - Preferred
  • Performance improvement experience - Preferred
  • Demonstrate mastery in computer skills in a Microsoft Windows based environment, i.e. Word, Excel, Power Point
  • Demonstrate proficiency in statistical software/tools and data analysis
  • Experience in process improvement models, e.g. Improvement Model, PDSA, Lean-six sigma, etc., meeting facilitation, and process improvement tools - Preferred
  • Must have a strong ability to cultivate and support a team environment that is cross-sectional and multi-disciplinary
  • Knowledge of infection control and prevention principles
