Description

Altoona Community Health Center is currently hiring a Housing Coordinator for their Transition from Homeless department.  This individual will provide services to an assigned caseload not to exceed 30 individuals. This will be done in a manner consistent with efficiency, cost effectiveness and regulatory compliance.

 

This position is Monday-Friday, 8am-4pm.


Responsibilities:

  • Assist in the completion of reports required by funding sources.
  • Collaborate with area housing, treatment and social service agency professionals. Collaborate with area housing agencies and landlords to promote permanent housing for clients. Plan and provide services in close cooperation with case managers within the behavioral health system. Market the Blair County Project for Assistance in Transition from Homelessness (PATH), Homeless Assistance Rental Project (HARP), and Homeless Prevention in Rapid Rehousing (HPRP) services to professionals in a position to refer homeless clients. Participate in meetings and committees related to housing, including the Local Housing Options Team (LHOT).
  • Collect individual and housing related data for reporting purposes.
  • Demonstrate support of Agency mission and philosophy in delivery of services.
  • Endeavor to achieve and maintain quality and customer satisfaction in the delivery of services to internal/external customers.
  • Maintain and protect patient, individual and Agency confidentiality.
  • Participate in educational programs, quality assurance, and quality improvement activities.
  • Promote personal and co-worker safety during work duties.
  • Provide services to an assigned caseload not to exceed 30 individuals. Identify housing needs of homeless individuals through outreach in the community. Establish service plans with individuals to focus on housing. Implement service plans in cooperation with the individual, family and other professionals. Evaluate the individual's progress based upon the service plans at required times and as special needs arise. Revise service plans in cooperation with the individual and supervisor. Advocate for individuals in securing necessary services and resources to meet their needs. Provide life skills training to enable individuals to adapt to a new housing situation. Provide supportive counseling to enable individuals to make appropriate life choices. Communicate effectively regarding individual progress through reports, records, and team conferences.
  • Provide services which will assist individuals in attaining the following outcomes: Screen potential individuals for PATH and HARP eligibility. Screen potential individuals for HUD eligibility. Assist individuals in accessing services through Blair County Housing Authority (COBRAH). Assist individuals in contacting landlords and screening apartments for residency. Assess homeless individuals to determine appropriate housing placement. Assess individuals to determine needed mental health services. Assist individual in the completion of housing related paperwork. Locate resources to help individuals obtain permanent housing. Refer and link individuals to relevant and needed community services. Provide follow up case management to assist consumers in remaining in permanent housing. Provide mentoring services to Homeless Housing Assistant.
  • Represent and participate in Agency professional and community activities as requested.

Qualifications

  • Bachelors Degree in related field required.
  • Minimum of one year related work experience.
  • Competency in computer skills in a Windows based environment.
  • Access to transportation in the Agency service area with valid Pennsylvania drivers license and auto liability insurance.
  • Availability for evening work.
