UPMC Altoona - Family Physicians - Clinical Pharmacist - Fulltime - Daylight - Monday-Friday

 

Purpose:
Ensure provision of cost-effective pharmacotherapy contributing to optimal patient outcomes, provide experiential education for present and future health care practitioners, and support drug-related research. These services will be provided in compliance with federal and state regulations; JCAHO standards; and health system, hospital and department policies. Support a variety of patient populations, ranging from geriatrics to pediatrics. Inpatient and outpatient practice areas include, but are not limited to general medicine, surgery, intensive care, transplantation, orthopedics, oncology, and cardiology.

Responsibilities:

  • Complete continuing education programs related to Pharmacy Practice as a mandatory requirement for maintaining an active license in the Commonwealth of Pennsylvania (requirement = 30 hours/2 years).
  • Mentor pharmacy students and/or residents to facilitate ongoing teaching programs and support Department and School of Pharmacy missions.
  • Participate in development and implementation of new initiatives by reviewing literature, prepare educational materials, determine implementation strategies, design informatics, create outcome measures and monitor forms, and/or design competency assessment tools.
  • Participate in discussion of literature review for evaluation of place in therapy for new drugs or disease management concepts, identification of new opportunities, and for reviewing implemented initiatives for current content.
  • Prepare special projects, presentations (national or local), seminars, QI Fair entries etc.
  • Provide educational in-services to pharmacy, nursing, and/or medical staff related to DUDSM Program initiatives or clinical programs.
  • Provide staff training and support of a decentralized pharmacy model.
  • Review medication orders for safe and effective indication, dose, route, and scheduled administration.
  • Screen medication orders for adverse drug events (ADEs) and medication errors (MEs) as per hospital policy. Report ADEs and MEs through approved reporting mechanisms.

Qualifications

  • B.S. degree or PharmD in Pharmacy from an accredited School of Pharmacy.
  • 2 years experience in hospital pharmacy or a one-year accredited residency
  • Good written and verbal communication skills.
  • Ability to work well independently, and demonstrate leadership in their current position.
  • Completed Residency in Primary Care is preferred

    Licensure, Certifications, and Clearances:
  • Act 34 Criminal Clearance
  • Pharmacist

Salary Range: $43.62 to $69.38
