Description

Purpose:
Manage the coordination and supervision of the direct patient care services. Work with the Nurse Manager and Director of Operations in planning, scheduling, and directing the provision of clinical services.

Responsibilities:
  • Ability to train staff in productivity expectations.
  • Act as a resource person to the community.
  • Assist Nurse Manager in planning/implementing orientation program for all clinical staff.
  • Assist in taking new referrals and completing intake appropriately and timely.
  • Assist the Nurse Manager in hiring qualified patient care staff.
  • Assist the Nurse Manager in meeting and training staff regarding JCAHO standards and applicable state and federal regulations.
  • Assist the Nurse Manager in planning and implementing case conferences and clinical meetings. Participate in all center staff and clinical meetings.
  • Assure provision of quality clinical services in the home.
  • Complete performance evaluations of clinical staff in a timely manner.
  • Coordinate and integrate with all disciplines in the delivery of patient care.
  • Coordinate with Pharmacy staff in setting up new referrals, supplies/equipment and deliveries.
  • Coordinate with other center staff in assuring ongoing third party case management authorization/verification procedures are followed.
  • Coordinate, assign, and schedule patient care services.
  • Demonstrate a self directed and disciplined approach to job.
  • Demonstrate the ability to remain calm under pressure.
  • Develop and maintain an effective level of communication with staff.
  • Display a neat, clean, professional appearance at all times.
  • Encourage open communication and verbalization of opinions among staff.
  • Ensure patient care is delivered in accordance with physician orders and policies and procedures.
  • Exhibits an attitude that promotes harmony and goodwill among clients and co-workers.
  • Give high priority to customer satisfaction.
  • Guide and counsel staff consistently.
  • Handle client complaints in a timely manner.
  • Hold clinical staff accountable for responsibilities inherent in their position.
  • Maintain confidentiality of information.
  • Maintain knowledge and adheres to all company policies and procedures.
  • Maintain knowledge of company forms and documentation process required for clinical services.
  • Maintain professional and technical job knowledge through appropriate resources.
  • Make joint visits with clinical staff to determine appropriateness of plan of care, assist with problem solving, training on new equipment or techniques, and evaluation of appropriateness of patient care.
  • Participate in orientation of new Nurse Clinicians and Intake Coordinators.
  • Plan and organize work effectively.
  • Prioritize responsibilities and workload.
  • Promote Chartwell culture by providing quality, comprehensive services to all customers through a team approach.
  • Provide direct patient care as needed.
  • Provide for in-service training for clinical staff in conjunction with th Nurse Manager.
  • Provide ongoing, clear and measurable performance expectations for clinical staff.
  • Serve as a resource to all clinical staff when questions or problems arise in provision of care to patients.
  • Support and train clinical staff in case managing responsibilities of patient care.
  • Support sales staff efforts by providing educational programs and participating in team selling.
  • Train clinical staff on documentation requirements for quality and timeliness.
  • Use appropriate, structured interview techniques and selection criteria.

Qualifications

  • Previous training and experience in home health high-tech and AIDS/Oncology clinical areas. 
  • 2 years experience in home infusion/home health care, CPR certification, excellent interpersonal communication and organizational skills. 
  • 1 year recent supervisory experience preferred.

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Act 73 FBI Clearance
  • Automotive Insurance

  • Cardio Pulmonary Resuscitation
