Description

Purpose:
Provide nursing guidance via phone to patient/family/caregiver as prescribed by the physician and in compliance with all applicable laws, regulations, and company policies and procedures. Follow the plan of care to meet patient/family/caregiver needs. Provide timely documentation of patient services based upon the plan of care. Identify changing needs and communicate those with RN Case Manager. Work in collaboration with the Director of Clinical Services and the interdisciplinary team in the planning, implementation and evaluation of medical and nursing care. Act a role model, providing leadership and support to the hospice staff. Manage related expenditures in a fiscally responsible manner in accordance with the company budget, travel as necessary.



Responsibilities:

 

  • Seek and obtain sufficient information necessary to function effectively in the setting where the patient/family/caregiver require care.
  • Conduct assessment, including physical and psychosocial aspects of patient/family/caregiver in a thorough, accurate, compassionate and timely fashion.
  • Perform emergency visits, if needed. Contribute to identifying both active and potential needs of the patient/family/caregiver which may affect the plan of care.
  • Consistently collaborate with the physician to comply with the medical and nursing plan of care for any after hours problems.
  • Review the plan of care with the interdisciplinary team, documentation in the EMR with email follow up with the team to report on the previous night’s concerns/interventions.
  • Collaborate with after hours staff to provide continuous assessment of assigned patients, interpret findings, initiate appropriate therapeutic actions and evaluate the results of these actions.
  • Perform within the limits of preparation, training and experience.
  • Work in cooperation with the patient/family/caregiver and the interdisciplinary team to meet their emotional needs.
  • Teaching and education of patient/family/caregiver to promote continuity of care and optimal outcome for quality death as defined by the patient/family/caregiver.
  • Assists in the investigation and resolution of quality issues in conjunction with the Total Quality Assurance committee.
  • Documentation in McKesson with accurate and relevant clinical notes regarding patient’s condition and care given for problems that occur after hours.
  • Act as a mentor to staff development, including participation in the orientation of new personnel and assist with field training as needed.
  • Communicate clearly and appropriately to patient/family/caregiver, physicians, nursing facility staff and other referral sources, relating all pertinent information in written and verbal forms. Maintain open communication with supervisory personnel.
  • Regular attendance is to be maintained.
  • Maintain electronic records, statistics, reports and records for reporting and evaluating purposes in compliance with local, state and federal laws.
  • Participate in staff meetings, department meetings, team meetings, briefings, in-services, committees and other related activities as needed.
  • Assure compliance with local, state and federal laws, Medicare regulations and established company policies and procedures.
  • Meet or exceed delivery of company service standards in a consistent manner.
  • Conduct all business activities in a professional and ethical manner.
  • Maintain all appropriate state nursing licensing requirements


Qualifications

Education:

  • Registered Nurse required.

Experience:

  • Two years of related work experience.
  • Prior experience in a customer-focused environment preferred.
  • Computerization Skills: Computer skills in a Windows based environment. Travel: Transportation to an alternate site may be required in the event of a facility emergency.
  • Schedule: Varying schedule to ensure adequate coverage of the Customer Contact Center 365 days a year.

    Licensure, Certifications, and Clearances:
    Certification/Licensure: Registered Nurse license required. CPR preferred

  • Act 34 Criminal Clearance
  • Registered Nurse
