Purpose:

Perform daily activities of medication order entry and IV admixture preparation and checking. Clarify invalid and unclear prescription orders including variances from defined standards and interventions such as drug interactions and dosage. Oversee workflow, maintain required documentation, and assist in the development and testing of staff competencies. Assist in the development and implementation of new policies and procedures.

Responsibilities:
  • Assist in filling vacant shifts and call offs.
  • Assist in the development and testing of staff competencies.
  • Insure timely billing of medication.
  • Maintain compounding logs, inspection logs.
  • Maintain involvement in drug interventions and monitoring as defined by the Department including the review of dosing regimens for selected agents, recommendations for monitoring patients and suggestions for dosage modification when necessary.
  • Order or borrow products to meet hospital emergent needs.
  • Oversee departmental workflow.
  • Oversee medication kit replacement system.
  • Participate in the design and implementation of drug use evaluations, and is involved in data collection and clinical intervention when appropriate.
  • Perform the drug dispensing and IV preparation activities and ensures the efficient and accurate delivery of medications to patient care areas.
  • Retrieve drug information from Micromedex, standards texts, and other sources of information and applies it to general patient care questions.

Qualifications

  • Bachelors or Doctorate degree from an accredited School of Pharmacy. 
  • Five years of experience in hospital pharmacy preferred.  
  • Ability to obtain and interpret information in terms of the specific needs of the patient served. 
  • Knowledge of drug dispensing and IV preparation policies and procedures. 
  • Understand the range of treatment needed by the patients served.

Licensure, Certifications, and Clearances:
  • Act 34 Criminal Clearance
  • Pharmacist

Salary Range: $42.89 to $70.84
