UPMC Bedford - Clinical Pharmacist - Part time - 16 hrs a week - Daylight shift with potentially evening shifts

 

 

 

Purpose:
Coordinate clinical pharmacy services and ensure that the pharmacy provides optimal clinical services on a daily basis. Facilitate the development, implementation, and maintenance of systems that promote desirable patient outcomes through the use of safe, cost-effective drug therapies.

Responsibilities:

  • Document activities through departmental reports and quality assurance mechanisms.
  • Maintains expertise in area(s) of practice through continual learning experience.
  • Participate in and support Pharmacy and Therapeutics Committee Activities such as drug use and disease state management initiatives, adverse drug reaction monitoring and formulary review and maintenance.
  • Perform pharmacotherapeutic monitoring of patients in area of service responsibility in a routine, continuous manner taking into account the age and clinical condition of each patient.
  • Provide and document pharmacotherapeutic recommendations that positively affect patient outcome.
  • Provide general and patient-specific drug information to patients, physicians, nurses and other healthcare professionals.
  • Serve as experiential rotation preceptor for pharmacy students and post-graduate trainees.
  • Support other staff pharmacists in providing pharmaceutical care to patients.Interact routinely with physicians and nurses to provide optimal pharmaceutical care to patients.
  • Support pharmacy-related research.

Qualifications

  • Pharm.D. or BS degree from accredited School of Pharmacy
  • Hospital experience preferred
  • Must exercise care in handling sensitive drugs and solutions.
  • Ability to receive, comprehend, and carry out instructions.
  • Ability to work safely with equipment, supplies, and materials.
  • All certified technicians and technologists are required to provide periodic evidence of continued registry in their respective professional association.
