The Jameson Child Advocacy Center is currently hiring a Full-Time Nurse Home Visitor to support the Nurse Family Partnership program at our New Castle, PA location.

This position will require you to attend a week long training in Colorado. Shifts for this position will be primarily daylight shifts, with 100% travel (reimbursed). This position is home-based, with 25-27 cases. This position will not require on-call. 

 

Nurse-Family Partnership home visitation program for first-time mothers and their children in Lawrence County. Assess the needs of the women and the child/children. Provide education, support, and referral sources. Document program activity. Perform case management. Collaborate with other NFP team members, county agencies and professionals.

Responsibilities:

  • Ability to locate and activate family resources.
  • Maintains consistent contact with referral source and provides regular feedback to referral source regarding services provided.
  • Provides and maintains the required units of service as determined by the Executive Director.
  • Assists with new programs or changes in existing programs based on current or projected community needs. Assists with recommendations based on individual client needs, enabling them to access the most appropriate level of care for their situation.
  • Attends continuity meetings to promote a positive working relationship and mutual understanding among local providers.
  • Meets with designated staff or affiliated organizations as needed to maintain and promote a continuing spirit of cooperation.
  • Assumes personal responsibility for professional development and continuing education; discusses personal goals for continued development with the Executive Director.
  • Available for crisis situations as necessary and provides back-up services to other team members.
  • Maintains client confidentially.
  • Utilizes release forms as necessary.
  • Participants in interdisciplinary efforts to achieve effective case management.
  • Demonstrates the ability to independently solve problems in a proactive and creative manner. Maintains good physical and emotional health necessary to fulfill the responsibility of the position. Orders supplies as appropriate.
  • Identifies problem areas and makes suggestions and recommendations to resolve problems or enhance development. Assists with recommendations based on clinical considerations, financial feasibility and existing resources. Assists with training and consultation to potential referral sources.
  • Maintains working knowledge of assigned families in the county, and oversees case management activities.
  • Provide intake, assessment, home visits and service coordination for clients referred to the Nurse-Family Partnership Program.
  • Follows established procedures of the NFP Program.
  • Prepares client assessments by interviewing client and family. Identifies client and family strengths and problem areas. Schedules initial home visits in a timely fashion and provides follow through with family services.
  • Participants in the development of the individualized action plan. Includes families in the development of the individualized action plan.
  • Provides the Executive Director with accurate, complete and timely information as it relates to patient care, personnel issues and other program activities.
  • Routinely maintains client notes and progress reports as required by NFP policies and procedures.
  • Assists with planning, development and implementation of new programs consistent with the changing treatment environment and the agency's strategic imperatives.
  • Recognizes and evaluates personal strengths and weaknesses and requests assistance in situations that exceed abilities or authority.
  • Adapts to and supports changes in the agency functions, management styles, or unit operations.

Qualifications

  • OBGYN experience strongly preferred
  • Pediatric experience strongly preferred
  • Minimum 6 months experience.
  • BSN strongly preferred.
  • If no BSN, those with a bachelors degree with nursing experience will be considered
  • Previous nursing experience in pediatrics or maternity strongly preferred
  • The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below.
  • They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures.
  • Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.
  • Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment.
    Good clinical judgment with critical thinking, analytical and problem solving abilities required as related to various aspects of patient care.
  • Critical thinking skills necessary to exercise and to lead others in application of the nursing process.
    Mobility and visual manual dexterity.
  • Physical stamina for frequent walking, standing, lifting and positioning of patients.

Licensure, Certifications, and Clearances:

  • Registered Nurse
  • Cardio Pulmonary Resuscitation
  • Act 33 Chld Clearance with Renewal
  • Act 34 Crim Clearance with Renewal
  • Older Adult Protective Svs Act OR Act 73 FBI Clearance
  • UPMC approved national certification preferred.
  • Current Pennsylvania licensure as a Registered Professional Nurse.
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

Salary Range: $15.38 to $24.59
