UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. The Jan. 13 Supreme Court of the United States decision that the Centers for Medicare & Medicaid Services federal COVID-19 vaccine mandate will move forward requires UPMC to ensure employees either get vaccinated or receive a requested medical or religious exemption.

RN Care Manager, PACT


UPMC is hiring an RN Care Manager, PACT!

This grant-funded position will allow you to be a part of medical case management, take ownership of the new telemedicine initiative (includes mobile telemedicine), and spearhead the HIV and STD testing initiative. This AIP covers 4 counties (Blair, Bedford, Huntingdon, and Fulton), so this position will include travel. You will find yourself building relationships with clients unlike any other, have the opportunity to build two new programs, and be able to join a highly engaged team! You will find that there is a ton of room for growth in the position, as well as flexibility within your schedule.


  • Conduct comprehensive face-to-face assessments that include the medical, behavioral, pharmacy, and social needs of the client. Review documentation in the client's electronic health records as appropriate and identify gaps in care based on clinical standards of care. Refer clients to appropriate case management, health management, or lifestyle programs based on assessment data. Engage clients in education or self-management programs. Provide clients with appropriate education materials or resources to enhance their knowledge and skills related to health or lifestyle management.Contact clients with gaps in preventive health care services and assist them to schedule required screening or diagnostic tests with their providers. Assist client to schedule a follow-up appointment after emergency room visits or hospitalizations.
  • Review client's current medication profile: identify issues related to medication adherence, and address them with the client and providers as necessary. Refer client for Comprehensive Medication Review as appropriate.Successfully engage clients to develop an individualized plan of care in collaboration with their primary care provider that promotes healthy lifestyles, closes gaps in care, and reduces unnecessary ER utilization and hospital readmissions. Coordinate and modify the care plan with the client, caregivers, PCP, specialists, community resources, behavioral health, and other specialists as appropriate.Present or contribute to complex case reviews by the interdisciplinary team summarizing clinical and social history, healthcare resource utilization, and case management interventions. Update the plan of care following review and communicate recommendations to the client and providers.
  • Assist client with the transition of care between health care facilities including sharing of clinical information and the plan of care.Provides phlebotomy services to include HIV rapid and confirmatory testing, STD/STI testing, Hep C screening, and another testing as requested.Facilitates telemedicine visits with clients and other HIV-positive individuals. Provides in-home telemedicine visits for clients.Utilize appropriate project resources to meet client needs for housing, medication, transportation, and nutrition. Transports clients to appointments following Ryan White guidelines.



  • Minimum of 2 years of experience in a clinical setting and case management nursing required.
  • BSN required.
  • 1 year of experience in clinical, utilization management, home care, discharge planning, and/or case management preferred. 

Licensure, Certifications, and Clearances:

  • Case management certification or approved clinical certification is required (or must be obtained within 2 years of hire to remain in the role)
  • CPR is required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
  • Automotive Insurance/Driver's License
  • Act 34 with renewal
