Description

Great Lakes Home Health and Hospice is currently hiring a Full-Time Hospice Aide. This position includes home visits for both home health and hospice patients in the Erie County area.

This is a Monday through Friday, 8:00 AM to 4:30 PM position with rotating weekend shifts.

 

Responsibilities:

  • Participate in all required agency meetings.
  • Identify, develop and participate in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Communicate appropriately with the interdisciplinary IPU care team in a timely manner, reporting any changes in the patient's condition to the appropriate professional staff.
  • Provide safe and direct personal care to patients in the IPU setting under the direct supervision of appropriate professional
  • Fulfill agency responsibilities by completing all required forms accurately and in a timely manner.

 

Qualifications

  • High School Diploma or GED - Required
  • Must have successfully completed a training and competency evaluation program as specified in Home Health and/or Hospice as applicable
  • One (1) year experience - Preferred 
  • Certified Nurse Aide - Preferred
  • Must maintain annual competency evaluation scores as determined by regulatory requirements.


Licensure, Certifications, and Clearances:

  • Automotive Insurance
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Certified Nursing Assistant (CNA) - Preferred
  • Driver's License
  • Act 33 with renewal
  • Act 34
  • OAPSA
