UPMC Western Maryland is seeking a part-time Care Coordination Assistant to support our Care Coordination Departmet!   This position will work during daylight hours! This is a great opportunity for someone who is customer and patient-care-focused! 


  • Provides support to the Care Coordination Department assisting with the discharge and utilization management process.
  • Performs tasks as delegated by the Care Coordination staff during the initial and ongoing phases of discharge planning for the management of patients across the continuum.
  • Provide ongoing follow-up and basic interviews with patients including enrolling and or referring patients to community resources.
  • Act as a patient advocate and liaison between the patient/family and community service/resource agencies to improve the health of patients and families. Assist patients with completing applications, and registration forms, and connect with transportation resources.
  • Schedule follow-up primary care provider appointments, provide appointment reminders and make follow-up phone calls when appropriate.
  • These duties will include both clerical and non-clerical functions that are within the scope of non-licensed personnel within the inpatient and ED/Observation Unit setting.
  • Ordering durable medical equipment, arranging transportation, setting up post-discharge appointments, issuing the Important Message from Medicare letters to patients, providing Advanced Directive Information, and assisting patients with completion as needed.

  • High School Diploma/GED required.
  • Customer service and/or previous work in clinical setting preferred.
  • Understand the community served and be knowledgeable about community resources appropriate for needs of patient/families.
  • Ability to provide information to patients as delegated while monitoring and reporting on any need for follow up by licensed staff.
  • Basic computer skills including Word, Excel, and Outlook.
  • Excellent communication, organizational and prioritization skills.
  • Motivational interviewing to support challenging discharge planning situations.
  • Ability and willingness to provide emotional support, encouragement and motivation to patients as well as the ability to initiate and maintain positive working relationships with staff and other organizations.
