Description

Join UPMC Home Health and provide critical one-on-one nursing care to patients. This unique role is for our resource pool that floats to patient home locations for the team while receiving a premium shift differential of 10%.

Job title and salary will be based upon qualifications and career ladder requirements.  This position is Monday - Friday daylight, on-call, and weekend rotations.

Responsibilities:

  • Provides direct patient care, evaluates outcomes, and adjusts the nursing care process as indicated to ensure optimal patient care. Initiates coordinates and evaluates patient and family-centered health teaching. Establishes the plan of care in collaboration with the physician, and patient. Advises and consults with family and other agency personnel, as appropriate.
  • Completes documentation including visit assessment, OASIS, 485, and Verbal Orders accurately and promptly on the laptop device or in written documentation while in the patient's home and within the agency and regulatory standards. Data syncs (transmits) information same day.
  • Exercise independent judgment in matters concerning emergent and non-emergent patient care needs and communicate with the physician.
  • Coordinates case management activities within a multi-disciplinary health care team to insure appropriate delivery of services to patients. Initiates, coordinates and insures ongoing communication with the physician and other members of the health care team. Role models assertive and professional communication strategies and holds peers accountable for similar behavior.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. 

 

Qualifications

  • Graduate of an accredited school of professional nursing required
  • Six months of experience in Acute Care, Skilled Care Facility, or HH/Hspc Nursing required.
  • BSN - Preferred UPMC Visiting Nurses Association approved certification - Preferred One year of Home health experience.
  • Preferred annual performance rating of good, solid, strong performance or higher on the most recent EPR
  • Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.
  • Ability to work productively and effectively within a complex environment, and handle multiple/changing priorities and specialized equipment.
  • Good clinical judgment with critical thinking, analytical, and problem-solving abilities are required as related to various aspects of patient care. 
  • Mobility and manual visual dexterity. Physical stamina for frequent walking, standing, lifting, and positioning of patients.
