UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. The Jan. 13 Supreme Court of the United States decision that the Centers for Medicare & Medicaid Services federal COVID-19 vaccine mandate will move forward requires UPMC to ensure employees either get vaccinated or receive a requested medical or religious exemption.

If you have a compassionate personality and desire to help individuals and family’s complete the circle of life, then hospice may be a fit for you.  Helping to meet the needs of individuals who are dying can certainly become a gift of life.  
Hospice looks not only at dying with dignity but also how we can help those living.  We are always seeking ways to improve lives of our families through bereavement, providing patients opportunities to bring closure to their own lives/relationship. 
We encourage certification in Hospice/Palliative care SW, Bereavement, and desire to mentor new SW in the field of End-of-Life care through local universities/colleges.  Some weekend shifts and flexibility may be needed with this role due to patients' needs. 

Responsibilities:

  • Maintain extensive knowledge of federal, state and local assistance programs and community resources that affect patient needs.
  • Perform initial and comprehensive assessment of patient/family
  • Facilitate end of life decision making including familial and financial stressors
  • Develop, coordinate, and update individualized plan of care with IDG addressing mental, psycho-social and social determinants related to care
  • Possess effective communication skills to assist and guide patient/family and IDG

Qualifications

  • MSW degree from a CSWE accredited School of Social Work required.


Licensure, Certifications, and Clearances:
Current / active Social Work License required or successful pass of the social work licensing exam within 9 months of employment.

  • Cardiopulmonary Resuscitation (CPR)
  • Clinical Social Worker (CSW) OR Licensed Social Worker (LSW)
  • Driver's License
  • Act 33 with renewal
  • Act 34
