Description

This full-time position performs a vital role by supporting GLO Harrisburg’s medical case management services and HIV prevention efforts for young LGBTQ+ men of color and transgender women of color. Please note, this staff member must have the ability to work afternoon and evening hours at GLO Harrisburg (until 6:00 PM) at least one day per week. This staff member will work at the REACCH Program (located 1 mile away from GLO) from 8:30AM – 12:30 PM then transition to GLO from 1:00-5:00 PM the other days of the week. This role directly supports the GLO Harrisburg and REACCH teams by delivering high-quality HIV Navigation Services to individuals at a high risk of acquiring HIV. This role will include performing rapid HIV testing, collection of STI tests, and linking individuals to pre-exposure prophylaxis (PrEP) services. This individual may accompany clients to their medical appointments at the REACCH Program, HOPE Program, and/or other community agencies.  This individual must have excellent attention to detail skills and be able to fully assess situations and prioritize follow-up. Detailed documentation, and an ability to complete data reporting is a requirement for this position. The expected caseload for this position is up to 20 clients for HIV Navigation Services, and 20 clients through Medical Case Management services. Ample training and shadowing will be provided by the REACCH Program, the Pennsylvania Department of Health, the GLO Harrisburg team, and the program’s fiscal intermediary.  This position will report directly to the REACCH Program’s LCSW Case Manager Supervisor. Supervision will be provided for a minimum of one hour per week.

*This position does require at least 1 evening per week (until 6:00PM)*

Responsibilities:

  • Provide comprehensive medical case management services to a caseload of approximately 20 individuals living with HIV.
  • Perform and follow-up according to Part B Ryan White medical case management standards for these clients.
  • Ensures all clients applying for housing assistance meet eligibility and financial requirements
  • Provide HIV Navigation Services to approximately 10-20 clients in the form of linkage to pre-exposure prophylaxis (PrEP), post-exposure prophylaxis (PEP), and other risk-reduction activities.
  • Follow-up with clients to ensure they have access to PrEP or PEP medications and encourage adherence.
  • Document all interactions via client’s electronic health record within 5 business days.
  • Provide rapid HIV and STI testing on-site at GLO Harrisburg.
  • Serve as a resource to GLO staff and participants for community resources and referrals.
  • Provide HIV, STI, PrEP, and other sexual health psychoeducational groups at GLO Harrisburg at least monthly.
  • Participate in community outreach events for education and HIV testing alongside the GLO and REACCH teams.
  • Complete other tasks as assigned.

Qualifications

 

  • Master’s degree in social work, counseling or related field required.
  • An ability to demonstrate one’s commitment to social justice and experience serving marginalized populations, such as LGBTQ+ and/or BIPOC clients preferred.
  • Interest in community-based case management preferred.
  • Experience providing case management and knowledgeable about community resources in Dauphin county preferred.
  • Demonstrated experience exhibiting exceptional organizational, assessment, teamwork, and critical thinking skills.
  • Demonstrated ability to motivate and work with folks from diverse backgrounds.
  • Ability to navigate resources and work independently serving case management clients.
  • Bilingual preferred.
  • Experience in the HIV/AIDS field preferred.
  • Understanding of and the ability to apply motivational interviewing, solution-focused, and strengths-based client-focused techniques preferred.


Licensure, Certifications, and Clearances:
Current / active Social Work License required or successful pass of the social work licensing exam within 9 months of employment.

Clearances must be dated within 90 days

  • Clinical Social Worker (CSW) OR Licensed Social Worker (LSW)
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
