Description

Are you a Registered Nurse, RN, with home health, public health, or Nurse Family Partnership experience looking to take your career to the next level? Nurse Family Partnership (NFP) is looking for a Supervisor for their team. This is an additional position for this expanding program. In this position, the NFP Supervisor works both in the office and out in the community. The supervisor will travel within the service area to various sites including, but not limited to, the other NFP offices and client's houses. 

Hours: Primarily Monday-Friday dayshift, will discuss specifics further in interview.


The NFP Supervisor will use principles of supervision, reflective practice, and staff development, to provide primary support and appropriate oversight to the Professional Staff Nurse NFP, and to build and model a strengths-based, client-centered culture.

Responsibilities:

Basic Job Functions 

  • Work within the local community and Agency structure to assure that the Nurse-Family Partnership (NFP) is implemented in accordance with established NFP Visit-To-Visit Guidelines and contract requirements for designated service area/region.
  • Using principles of supervision, reflective practice, and staff development, provide primary support and appropriate oversight to the Professional Staff Nurse NFP, and to build and model a strengths-based, client-centered culture.
  • Provides home visits to women and their families eligible for NFP services.
  • Adheres to nursing process and the NFP model of home visitation.
  • Carries a pro-rated caseload according to number of NFP nurses supervised and other responsibilities.

 

Utilizes reflective practice in supervisory sessions - 

Schedule joint home visits with NFP Nurse Supervisor every four months.

Attend and participate in bi-weekly case conferences and team meetings.

Maintain quality, timely and appropriate comprehensive documentation both written and data entry into Efforts To Outcomes (ETO) software and Agency electronic health record.

Provide information to support staff for timely and accurate data input to the ETO.

Participate in review and analysis of ETO reports for achievements and areas for improvement.

Participate in quality improvement efforts.-Maintain confidentiality.

Promote safety awareness for client/family members in their environment.

Responsible for meeting/exceeding customer service initiatives.

Initiate activities to report and/or resolve customer complaints.

Adhere to infection control principles, standards, policies and procedures.

Perform related duties as assigned or required.

Community awareness, support, referral/referral systems (in conjunction with NFP Director):

Actively participate in all NFP community coalition/advisory board meetings as requested.

Develop and implement external and internal systems to assure timely receipt and disposition of referrals to and from the NFP.

Develop and implement marketing strategies to promote NFP and Agency, ensure adequate referral base and enhance community support of the program.

Develop and implement strategies according to Agency protocols for using local print and electronic media to showcase NFP.

Budget and Contracts

Become familiar with and actively participate in the preparation and monitoring of the NFP budget to ensure appropriate use of funds.

Plan and expend budget to ensure compliance with fidelity to NFP model and contract agreements.

Understand and actively participate in monitoring NFP contract with Agency

Proactive with local, state, and national issues that may affect sustainability of program.

Communication

Maintains clear, effective, open, honest communication with both internal and external customers and colleagues.

Creates, maintains and supports a safe environment for open discussion.

Seeks and responds appropriately to feedback.

Quality Improvement and Evaluation

Oversee and ensure timely and accurate data input and utilization of ETO reports and other reports available through NFP and Agency information systems.

Analyze NFP program reports both ETO and Agency; share appropriately with team and partners.

Ensure implementation of Quality Improvement and strategies.

Develop methods for ongoing assessment of staff development needs; compare available data to structure appropriate continuing education and guidance for staff.

Provide routine surveillance of potential client care/Significant Event incidents, as per Agency protocols.

Evaluate program outcomes and interventions, formulating corrective action plan where indication.

Conduct clinical record reviews on active clients as per Agency protocols for completeness, fidelity to model, and in line with contract and billing requirements.

Consults and collaborates with other professionals involved in providing services to women and families.

Formulates nursing diagnosis based on nursing assessment and client goals.

Evaluates client progress toward stated goals and NFP outcomes.

Plans home visits in accordance with client goals and outcomes.

Manage and prioritize own caseload for safe, effective care according to acuity/needs of clients.

Prioritize clients for emergency preparedness.

Maintain productivity standards established for the program.

Utilize nursing expertise and professional judgement to inform appropriate healthcare provider of any pertinent change in condition of client.

Actively engages in professional development to meet all NFP Nurse Home Visitor competency requirements.

Meet with NFP Nurse Supervisor weekly for clinical supervision.

Become familiar and comfortable with principles/concepts of motivational interviewing, coaching and reflective practice.

Develop and implement regular reflective practice supervision within the program and conduct field visits, both with frequency recommended by NFP.

Ensure that own reflective practice and ongoing professional/personal development needs are addressed.

Provide regular case conferences and team meetings as recommended by NFP.

Ensure rewards/incentives for clients/staff are aligned with goals of program and Agency protocols.

Effectively assist home visitors in analyzing complex problems and case situations of clients in varying degrees of acuity, with consistency and in accordance with nursing and Agency standards of care.

Supports policies, procedures, NFP Visit-to-Visit Guidelines and standards of NFP and Home Nursing Agency.

As requested/required, participates in community coalition in support of NFP.

As requested/required, promotes public awareness of NFP.

Develops and maintains community relationships to support client referrals.

NFP Infrastructure (in conjunction with NFP Director)

Utilizes Agency policies and procedure or develop needed policy and procedures to guide NFP implementation.

Work within Agency, budget and contract specification, ensuring that NFP staff have infrastructure of space, computers, furniture phones, supplies (for clients too), etc. in place as soon as possible upon site startup.

Develop and maintain appropriate material for the community, healthcare/human service providers and client.-Oversee the allocation and design of space for NFP materials to be stored.

Ensure that all NFP forms and other materials are ordered in a timely manner, organized and available for use by home visitors (also in conjunction with NFP Support Staff).

Develop an adequate budget for appropriate materials, gifts and incentives for clients, ordering supplies as necessary to maintain inventory (also in conjunction with NFP Support Staff).

Manage organizational structure and systems adequately so that nurses my stay focused on their clients.

Create and Maintain NFP Culture

Develop, maintain and nurture relationships based on trust, support and growth with team/professional colleagues and community members.

Ensure that supervision of NFP nurses utilizes principles and concepts of motivational interviewing, coaching, reflective practice (modeling therapeutic relationships, change theory, self-efficacy, client centered, strength-based and solution-focused approaches).

Staffing and Education:

Oversee the recruitment and hiring of all nurse home visitors, ensuring they can implement the NFP philosophy and model.

Ensure that all staff, including supervisor, attend all required NFP Initial and Continued Education sessions as outlined in the orientation materials and contract with the NFP National Service Office (NSO).

Assess and develop strategies to address knowledge and skill gaps of NFP nurses.

Provide adequate orientation, professional development opportunities and study time to remediate gaps in necessary knowledge and skill.-Assign client caseloads and monitor nurses' ability to build and manage caseload.

Provide useful developmental feedback, confronting and developing a plan with staff to address performance and resolve issues.

Build confidence and skill level of NFP nurses.

Provide both supervision and evaluation of NFP nurses including annual leave, annual performance review, disciplinary action, etc.

Understands, supports, and coaches others in the NFP vision, mission and model

Represents the NFP vision, mission and model in actions and verbally to both internal and external customers and colleagues.

Develops therapeutic relationships, utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment.

Performs home visiting in accordance with the NFP model and NFP Visit-to-Visit Guidelines.

Follows nursing process in implementing NFP into nursing practice.

Ongoing assessment of physical, emotional, social and environmental strengths and challenges of women and their families as they relate to the NFP domains from prenatal period until the child's second birthday.

Assists women and their families in establishing goals and outcomes which are individualized, client-specific, measurable and realistic.

Provides education, support and referral resources in assisting women and their families in attaining their targeted goals in the NFP domains.

Qualifications

  • BSN required, Masters Degree in Nursing or closely related field preferred
  • 2 years experience in providing maternal/child health, public health, home visiting or mental/behavioral nursing; program management and one-on-one supervision preferred; worked with diverse populations, communities and organizations preferred Position requires travel to various office sites
  • Must be able to work a flexible schedule including some evenings and weekends.
  • Must be able to travel by air to Denver, CO for education sessions.

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire. Registered, certified or licensed as required by state law for the profession.
