UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. The Jan. 13 Supreme Court of the United States decision that the Centers for Medicare & Medicaid Services federal COVID-19 vaccine mandate will move forward requires UPMC to ensure employees either get vaccinated or receive a requested medical or religious exemption.

The Heart Failure Nurse Navigator (RN) will work at our Harrisburg Hospital, BLM or new technology parkway offices. The Bloom office is dedicated to nurse visits. The HH office is within Harrisburg hospital and is shared for provider and nurse visits. The Mechanicsburg office is a new Heart and Vascular office and nurse appointments will occur in a shared space.

The Heart Failure Nurse Navigator is an important member of UPMC Central PA’s Heart Failure Program. The Heart Failure Program is nationally recognized, with certification in Advanced Heart Failure by the Joint Commission. The Nurse Navigator practices at the top of their license as an expert clinician, coordinator, counselor, and coach to individuals with heart failure.

HOURS: Monday-Friday 7:30am-4:00pm - The casual position will only be assigned to one location per day


  • Role responsibilities are within acute care and in an ambulatory setting.
  • The Nurse Navigator reports directly to an advanced practice nurse.
  • The HF Nurse Navigator works as part of a team yet must be self-motivated, innovative and a patient advocate.
  • Within the inpatient setting, the nurse navigators identify heart failure admissions
  • Participate in daily team huddles
  • Assure adherence to HF metrics
  • Support patient education
  • Facilitate a safe transition plan.
  • In the ambulatory setting, the nurse navigators offer independent nurse office visits
  • Participate in shared office visits with advanced HF providers
  • Assure ongoing coordination of care in the community
  • Eliminate barriers to care so that individuals can implement self-management skills
  • Develop patient/family specific goals.
  • Collaborates with the interdisciplinary team in guiding the patient and family in regard to diagnostic and treatment options.
  • Provides emotional support specialized patient care, support and educational needs.
  • Assumes the role of care coordinator and patient advocate throughout the process.
  • Uses all components of the nursing process, to provide comprehensive nursing care, support and education to patients and their families during screening and diagnostic tests, treatment, recovery, and rehabilitation.
  • Provides expert knowledge of specialty (i.e. Heart Failure) issues to health care professionals and patients/families.
  • Works closely with the interdisciplinary team to meet the education and development needs of the professional and ancillary staff.
  • Monitors changes in legislation and regulations to assure that effective actions are taken in a timely and consistent manner.
  • Demonstrates UPMC's commitment to service excellence by understanding the needs of patients, their families, physicians and co-workers and doing what is required the first time, every time by being responsible, sensitive, accurate, timely, coordinated and thorough
  • Assesses and addresses the educational needs of individual patients and members of the community as a whole, in regard to health issues with an emphasis on specialty practice.
  • Participates in the evaluation and development of patient education materials and provides individualized teaching based on needs of the patients and the community.
  • Provides staff development and serves as a clinical resource and consultant to members of the interdisciplinary team, identifying additional resources as needed.
  • Coordinates tests, treatments, referrals and appointments as necessary to meet the physical, spiritual and psycho-social needs of the patient and family. Provides clinical support and patient care during procedures.
  • Works as a team member to plan, develop, market and implement community health and wellness educational programming and health care services, with an emphasis on specialty care.


  • RN with Bachelor's Degree preferred
  • 3-5 years of current clinical experience (specialty desired depending upon population served i.e. Heart Failure or Cardiology)
  • Must be self-directed and possess strong leadership and communication skills.
  • Demonstrated knowledge and clinical competence in the area of specialty with an emphasis on wellness and disease prevention throughout the life continuum.
  • Demonstrates ability for high degree of analysis, critical thinking and independent clinical judgment.

Licensure, Certifications, and Clearances:

  • Registered Nurse currently licensed in the practicing state.
  • Current certification in specialty or obtained within one year
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Registered Nurse (RN)
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
