Discharge Specialist | Western Psychiatric Hospital

Description

Western Psychiatric Hospital’s Dual Diagnosis Unit is looking to add a Discharge Specialist to the team! Dual Diagnosis is an Adult Drug Abuse and Psychiatric Treatment Unit consisting of a 26-bed inpatient service. The focus is on treating adults with a primary psychiatric disorder and co-occurring chemical dependency problem. This truly multidisciplinary inpatient unit believes in working together as a team to achieve the best possible outcomes for our patients. The discharge specialists are a part of the Social Work department. This position works specifically with 4 Social Workers on our floor to make referrals and coordinate with outpatient providers.    

 

Purpose:
Work collaboratively with the social worker to facilitate discharge planning for patients hospitalized on the adult and aging inpatient units.


Responsibilities:

  • Communicate issues related to discharge, placement, and after care with social workers and other treatment team members.
  • Assist inpatient social workers in facilitating discharge, placement, mental health care and community resources for patients and families.
  • Assist inpatient social workers in completing necessary paperwork related to disposition planning.
  • Maintain collaborative relations with UPMC programs and with community agencies and organizations.
  • Assist in assuring appropriate releases of information are obtained for collaborative contacts.
  • Assist in copying and faxing appropriate clinical information related to continuity of care.

Qualifications

  • B.A. degree in a Mental Health Related field is required.
  • Knowledge of mental health and community resources preferred.
  • Experience with or knowledge of Dual Disorders treatment and outpatient services is preferred. 


Licensure, Certifications, and Clearances:

  • Cardiopulmonary Resuscitation (CPR)
  • Comprehensive Crisis Management (CCMC)
  • Act 31 Child Abuse Reporting with renewal
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
