UPMC is hiring a Registered Nurse for its Home Health department in Centre County and surrounding areas.

Home Health is a great opportunity for a registered nurse looking for flexible scheduling options and providing one-on-one individualized care to patients in their homes.

Purpose:

The Professional Staff Nurse is responsible for the delivery of skilled nursing services to patients in their homes.

If you’re a nurse who wants to provide one-on-one care to educate patients and family members, home health has an opportunity to provide individualized patient care. Home health allows flexibility in your day while providing hand-delivered care to our most vulnerable population.

Responsibilities:

  • Attend compliance training and adhere to the organization’s standards of conduct, policies, and procedures.
  • Focuses on customer service and continually strives to perform their job duties in a manner that will result in optimal patient satisfaction. Assigns other nursing personnel, including LPNs and Aides, to assist in care delivery.
  • Collaborates with a multi-disciplinary health care team to ensure positive patient outcomes. Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC Home Health Care towards patients, visitors, staff, peers, physicians, and other departments within the organization.
  • Provides direct patient care, evaluates outcomes, and adjusts the nursing care process as indicated to ensure optimal patient care. Initiates, coordinates, and evaluates patient and family-centered health teaching in collaboration with the physician.

 

Qualifications

  • Graduate from an accredited school of professional nursing is required.
  • Bachelor's degree preferred.
  • UPMC-approved certification preferred.
  • Minimum of 6 months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care required.Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients and their families.
  • Ability to work productively and effectively within a complex environment, and handle multiple/changing priorities and specialized equipment.
  • Demonstrates good clinical judgment, critical thinking, and problem-solving abilities.
  • Mobility and visual manual dexterity.
  • Physical stamina for frequent walking, standing, lifting, and positioning of patients.

Licensure, Certifications, and Clearances:

  • UPMC-approved national certification is preferred.
  • Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.
  • CPR is required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
