UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. UPMC is hiring a Full-Time, Community Nurse Liaison covering Dauphin, Cumberland, and Perry Counties.  This position will have some Remote Work From Home with a flexible schedule after successful completion of training.  The position is 8:00 am - 4:30 pm Monday - Friday.

Working in an assigned territory or facility, facilitates referrals to home health or hospice by gathering and interpreting medical data, meeting with patients and families, and working with case managers to ensure optimal transition from hospital or other facilities to home.

  • Using individual skills, cooperates with others, and accepts and provides constructive feedback despite any personal conflicts between individuals involved.
  • Works closely with clinical leadership in the assigned region to drive a vision of growth by focusing every team member on the needs and expectations of the referral community.
  • Focuses on ensuring an ideal customer experience for all referral sources.
  • Utilizes current agency and/or department specific software to complete assignments.
  • Keeps agency management informed of facility changes regarding bed capacity, key marketplace changes in providers or competition, new facility programs, and/or procedures.
  • Works with referral partners to provide appropriate alternatives for patients who do not meet criteria.
  • Responsible for establishing, growing, and maintaining relationships with facility-based referral sources.
  • Knows the features and benefits of the services provided by the agency and can articulate competitive advantages, specialty programs, eligibility guidelines, and covered services.
  • After receipt of a referral, initiates the intake process by collecting the required orders, clinical information, and documentation and completing the appropriate data entry in the software system and/or scanning/faxing to the intake area.
  • Conducts informational visits with patients and/or their families, as requested to discuss eligibility guidelines, services offerings, benefits/covered services, and next steps.
  • Facilitates a timely and safe transition of the patient to the home setting.
  • Functions as an educational resource for case managers, physicians, and hospital staff to determine appropriateness of home health and hospice services and suggesting which services may be of benefit to patients.



  • Graduate from an accredited school of nursing. BSN and/or BS in a health-related field preferred.
  • At least 2 years of experience in acute care, skilled nursing facility, or home health/hospice preferred.
  • Experience in marketing preferred.
  • Must have the ability to work independently, multitask and problem-solve.
  • Demonstrates professional oral and written communication skills.
  • Demonstrates the ability to establish and maintain effective working relationships with a variety of referral sources.
  • Must be proficient in the use of technology for data entry, communications, presentations, and tracking/reporting.

Licensure, Certifications, and Clearances:

  • CPR is required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
