Description

Responsibilities

  • You will attend compliance training and adhere to the organization’s standards of conduct, policies, and procedures.
  • Focus is on customer service and continually strive to perform your job duties in a manner that will result in optimal patient satisfaction.
  • You will assign other nursing personnel, including LPNs and Aides, to assist in care delivery.
  • Collaboration with multi-disciplinary health care teams to ensure positive patient outcomes.
  • You will perform your duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC Home Health Care towards patients, visitors, staff, peers, physicians, and other departments within the organization.
  • You will provide direct patient care, evaluates outcomes, and adjusts the nursing care process as indicated to ensure optimal patient care.
  • As well as initiate, coordinate, and evaluate patient and family-centered health teaching in collaboration with the physician.

Qualifications

  • Graduate of an accredited school of professional nursing is required.
  • Bachelor's degree preferred.
  • UPMC approved certification preferred.
  • Minimum of 6 months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care required.
  • Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients and their families.
  • Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment.
  • Demonstrates good clinical judgment, critical thinking, and problem-solving abilities.
  • Mobility and visual manual dexterity.
  • Physical stamina for frequent walking, standing, lifting, and positioning of patients.

Licensure, Certifications, and Clearances:

  • UPMC approved national certification preferred.
  • Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.
  • CPR is required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
