Description

UPMC is hiring a Program Specialist to support the Vocational Services team at Mon Yough. 

The incumbent is primarily responsible for the development, coordination, and implementation of consumer services and ISPs for a caseload not to exceed 45. Also, they are responsible for the specific duties listed below, under the direction of the Vocational services supervisor and manager.

The incumbent will be joining a dedicated, supportive team. The ideal candidate will be a caring team-player who has knowledge of individuals with disabilities (especially intellectual disabilities), strong time-management and organizational skills, and the ability to stay calm under pressure. 

The start date for this role will be December 5, 2022. The shift for this position is Monday - Friday 8:00 a.m. to 4:00 p.m.

Responsibilities:

  • To accurately develop and implement the ISP and related plans (Fade, SEEP, Behavior). To complete with professional or technical proficiency coordination and conduction of assessments, reconciliation meetings, and gathering and reporting of all required information and in compliance with Allegheny County regulations and the Department of Public Welfare regulations.
  • Provides team members with documentation of the reviews, as well as making them aware of the option to decline
  • Assists in monitoring, and evaluating services provided to participants
  • Provide direct care coverage, including evenings, as needed. Assist with production requirements as needed. Provide personal assistance as needed. Provide transportation as needed. Complete 24 hours of approved training annually.
  • Develops, coordinates, and implements provider services
  • Establish and maintain effective working relationships with all MYCS consumers, striving to exceed their expectations for service, quality, and delivery.
  • Instructs direct care staff in the content of the Health and Safety needs relevant to each individual.
  • Prepare and review consumer payroll sheets for submission to payroll department as needed. Write, implement, monitor, and report progress on specific vocational goals.
  • Demonstrates proper judgement and decision-making skills when dealing with consumers and staff following company/program policies, county and state regulations accordingly.
  • Attends, participates in, and reviews the development of the ISP and all updates and revisions
  • Coordinates /or completes assessments for no more than 45 individuals
  • Documents the review of the ISP and all updates/revisions with the individual served, and ensures this documentation is made available to the Supports Coordinator
  • Completes documentation to include monthly statistics report Completes necessary documentation for admissions, discharges, and referrals.
  • Reviews, signs and dates monthly documentation of individuals? participation and progress toward outcomes. Conduct random quality assurance chart checks quarterly.
  • Reports content discrepancies and changes related to needs to the Supports Coordinator. Acts as the Plan Lead in the absence of an assigned Supports Coordinator
  • Provides the assessment for the development of the ISP and all updates and recommended service/plan revisions
  • Establish and maintains productive work relationship in a manner that promotes a positive and cooperative team-oriented work environment.
  • Coordinate and complete the development of behavior management plans for individuals who do not have an authorized contracted Behavior Specialist indicated in their ISP.

 

Qualifications

  • Bachelors degree in Social Science/Human Services+ 2 yrs work experience working directly with individuals with developmental disabilities.
  • Education must be through an accredited college or university in Administration, Business, Special Education, Psychology, Public Health, Rehabilitation, Social Work, Speech Pathology, Audiology, Occupational therapy, Therapeutic Recreation, or other human service field, which includes at least 15 credit hours in human services.


Licensure, Certifications, and Clearances:

  • Act 73 if they have lived outside of PA in the past 2 years ACRE or CESP certification
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Comprehensive Crisis Management (CCMC)
  • Driver's License
  • Act 34 with renewal
  • OAPSA
