Description

Summary:
UPMC Home Health is hiring an Intake Supervisor for the team.  The position manages staff responsible for all our patient intake duties, ensuring timely processing of referrals and a focus on customer service. The position works remotely and supervises a remote team. Excellent opportunity for a nurse that wants to make a difference but doesn’t want to provide direct patient care.  Occasional travel required for meetings and/or training 

Ensures the seamless patient transition into Agency services by interfacing with hospital/facility staff, physicians, and families to coordinate and process patient referrals. Acts as an educational and consultative resource to the staff and referral source regarding home health and hospice services.

Responsibilities:

  • Functions as the clinical leader that supervises the referral management process for patient care from physicians, hospitals, insurance case managers and other sources in the community. Monitors the active worklist and assists in the prioritization of referrals based on patient needs/acuity.
  • Always responds to communication regarding potential referrals for services with an extremely customer friendly and professional service orientation. Applies professional expertise to accurately completing referral documentation as necessary to facilitate the admissions process.
  • Reviews referral documentation, including orders, and contacts referral source for clarification of orders or other documentation to determine clinical eligibility for home health and hospice care. Works closely with clinical leadership to facilitate discussion about referrals during the referral-to-admission process.
  • Works closely with Business Development team regarding missing referral documentation; escalates appropriately to ask for assistance. Understands that teamwork is a crucial part of our business, and can work well with colleagues, no matter the circumstances. Using individual skills, cooperates with others, and accepts and provides constructive feedback despite any personal conflicts between individuals involved.

 

Qualifications

  • Completion of an accredited School of Professional Nursing required. BSN preferred.
  • Two (2) years of experience in a hospice, home health, medical, surgical, or critical care area. One-year management experience preferred
  • Must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise. Excellent written, verbal communication, and interpersonal skills required. Adaptability and flexibility to change. Ability to handle multiple tasks in a fast-paced environment and to function independently. Must have strong leadership skills to facilitate efficient prioritization, delegation, and throughput of the referral management process. Ability to analyze issues objectively, provide workable solutions and implement these actions.
