Shift for this role is Monday - Thursday 12-5pm & Friday 12-4pm
Purpose:
Perform various duties to include, but not limited to, retrieval, analysis and completion of specified patient records. The analysis of the records for completeness is in accordance with Medical Staff Bylaws, entity specific policy, the Joint Commission on Accreditation of Healthcare Organization, and Department of Health Standards. The Analyst must be able to track physician specific deficiencies, and ancillary department deficiencies as applicable.
Responsibilities:
- Verify the episode being analyzed in the medical record has been placed in the proper chart format/ order.
- When applicable, ensure appropriate release of information procedures followed as required by UPMC policies, and State and Federal confidentiality regulations. This includes verifying and identifying authority of requestors of information prior to release of information. Assure mode by which information provided (stat fax, messenger-stat, next-day processing) as appropriate for the urgency of the request.
- Verify the documents being analyzed in the medical records to ensure that they belong to the correct patient.
- Maintain productivity standards of all activities and submit reports to the HIM Management as required.
- May assist with file area functions, including, but not limited to: Filing of loose sheets, filing of dictated reports, routing charts throughout the department, copy records as needed, correct duplicate medical record numbers in all appropriate systems, answering phones and assisting callers.
- Accurately identify and enter deficiencies in the Chart Deficiency system. Assure incomplete records are re-analyzed following deficiency completion and update the Deficiency System accordingly.
- If applicable, retrieves incomplete records for physician completion and maintains the order of the physician chart room as needed.
- Accurately route charts to the proper location, tracking accurately in the chart tracking system (when applicable). Accurately orient, and/or assist the physicians with questions regarding their deficiencies and the requirements for chart completion.
- Assure timely and accurate processing of all death and birth certificates and proper release of bodies in accordance with appropriate UPMC and Commonwealth of Pennsylvania Bureau of Vital Statistics regulations.
- Assist HIM Management with the delinquent medical records process in their absence. Complete suspension/ fining procedures as applicable.
Qualifications
- High school diploma or equivalent is required.
- Computer/keyboarding skills and working knowledge of office equipment operations.
- The ability to accurately relay information and communicate, both verbally and in writing, in a tactful and courteous manner with peers, department management, physicians and other healthcare professionals.
- One year previous medical record department related experience is preferred.
- Prior medical terminology education and previous exposure to terminal digit filing is preferred.
- General knowledge of Pennsylvania and Federal HIPAA regulations regarding proper release of information procedures is preferred.
- Prior exposure to document imaging systems and other healthcare computer applications is preferred.
- Knowledge of record completion requirements, medical terminology, anatomy/physiology, and analytical skills is preferred.
Licensure, Certifications, and Clearances:
- Act 33 with renewal
- Act 34 with renewal
- Act 73 FBI Clearance with renewal
