Do you have an active or previous Emergency Medical Technician (EMT) or Paramedic certification and at least one year of experience in a health-related field? Do you have a high school diploma or equivalent? If so, UPMC may have the perfect fit for you!
UPMC Health Plan is hiring a full-time Community Paramedic to support the Connect Community Paramedics department. This is a Monday through Friday daylight role and will be based out of the Allegheny and/ or Washington counties. Travel within Allegheny and/ or Washington County will be required.
The Community Paramedic is responsible for staffing the daily operations of the CONNECT- Community Paramedic Program. They will be responsible for connecting with vulnerable patients and introducing them to available social determinants of health resources that could improve patients’ lives while promoting healthy steps, disease management skills, care transitions, and other services.
Responsibilities:
- Manage and follow-up on individual patient caseload using patient-centered approach.
- Complete patient biopsychosocial assessments.
- Connect patients to appropriate and various social service programs in effort to reduce gaps in care.
- Conduct face-to-face, virtual, or telephonic patient interactions as appropriate.
- Professionally document all patient interactions within the Health Plan’s documentation system.
- Complete all training required within established timeframes.
- Interact with EMS personnel, hospital staff, and Care Management team members to coordinate patient care.
Qualifications
- Highschool diploma or equivalent required.
- Current licensure + Minimum of 1-year experience in public health, EMS, health screenings or other health related field
- Proficient in MS Word/Excel
- Proficient in documentation platforms and computer/virtual platforms.
- Ability to interact professionally and effectively with patients, management, and other staffing.
- Reliable transportation
- Ability to work early morning hours and weekend hours as necessary.
- Ability to work at least one 8-12 hour shift per week.
- Ability to work 40 hour week.
- Be willing to commute between Washington and Allegheny County.

- Automotive Insurance
- Driver's License
