Physical Therapist for Home Health Williamsport:
UPMC Home Health is hiring a Physical Therapist for the Williamsport and surrounding area. You will work Monday - Friday daylight with occasional weekends and holidays. Home Health allow you flexibility, independence, one on one with patients in the home setting.
Provide age sensitive professional flexibility, Independence, one on one with patients in the home setting. physical therapy services in the home and community setting, including assessment, intervention, treatment and instruction to individuals of all ages to maximize functional abilities and ensure optimal care.
Responsibilities:
- Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
- Continually develops professional skills by assuming responsibility for on-going continuing education.
- Evaluates, plans, and treats patients in an effective Physical Therapy Program including the use of modalities and assistive devices within the Standards of Physical Therapy Practice.
- Identifies develops and participates in process improvement opportunities within the Agency's Home Health or Early Intervention programs that will enhance the quality of services we provide.
- Assigns and supervises the Certified Physical Therapy Assistant, if applicable.
- Attend compliance training and adhere to the organization standards of conduct, policies and procedures.
- Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of UPMC towards patients, visitors, staff, peers, physicians and other departments within the organization.
- Maintains and completes all required documentation, orders, Point of Care or written, promptly and accurately within Agency and regulatory standards. Data syncs (transmits) information same day, if applicable.
- Advises and consults with the family, inter-intradisciplinary team members, physicians and other professionals, as appropriate, to ensure optimal patient care.
- Provide physical therapy services and participates in the development of a physical therapy plan of treatment including modalities, frequency and treatment in collaboration with the referring physician for patients from birth to geriatrics to assist patient in obtaining maximum functional independence within the standards of physical therapy.
Qualifications
- Graduate of a Physical Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association; or Is eligible for the National Registration Examination of the American Physical Therapy Association
- Or Two (2) years of appropriate experience as a physical therapist and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service, except that such determinations of proficiency do not apply with respect to persons initially licensed by a State or seeking initial qualification as a physical therapist after December 31, 1977 required. One (1) year of Physical Therapy experience to practice without supervision or less than one (1) year if practicing under supervision, i.e., ALF/PCH, required
- Prefer Home Health experience
Licensure, Certifications, and Clearances:
- Automotive Insurance
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Driver's License
- Physical Therapist (PT)
- Act 33 with renewal
- Act 34 with renewal
- Act 73 FBI Clearance with renewal
