Case Manager (RN) Innovative Homecare Solutions
UPMC Innovative Homecare Solutions team is hiring a full-time Case Manager for the team. This role will work Monday - Friday daylight shifts with occasional weekend/evening shifts. The role is located at In Home 1650 Metropolitan St 3rd Fl Pittsburgh, PA 15233. After successful completion of orientation and training period, there is opportunity for hybrid work schedule. This unique role will support our new initiatives, and you will be part of guiding the new Life Changing Medicine team!
The Case Manager ensures continuity and coordination of care for members/patients with care coordination, health care education, and self-management needs. The Case Manager identifies member/patient barriers and the whole needs of the members/patients including medical, behavioral, pharmaceutical, and social needs. Develops a comprehensive care plan that includes prevention, member/patient self-management education, and clinical standards of care. Coordinates with other medical management staff, other departments, providers, members/patients, and care givers to provide additional support to the members/patients.
Responsibilities:
- Provide case management according to program descriptions including member/patient education, follow up on a timely basis and eliminating barriers to care.
- Comfortable with change, innovation, and strategic thinking
- A person in this position needs to be comfortable with creative problem solving and have excellent communication skills. This role will be to communicate with various internal and external leaders.
- Founding nurse case manager for an innovative, home-based acute care model
- Provide clinical coordination for a multi-disciplinary team
- Support patients/members in their homes instead of an ED admission
- Autonomy to influence process and workflow improvements and grow the program
- Work with telemedicine physicians, field paramedics and EMTS, remote monitoring nurses
- Successfully engage members/patients to develop an individualized plan of care to promote healthy lifestyles, close gaps in care, reduce unnecessary ER utilization and hospital readmissions. Develop and coordinate the individualized plan of care with the member/patient, member's/patient's family, providers, and community agencies as applicable. Involve other staff to support the individualized plan of care based on identified needs of the member/patient
- Based on assessment of members/patients' needs and environment, identify applicable barriers, problems, goals, and interventions that will address those needs and improve or maintain the health status of the member/patient.
- Follow-up with the member/patient according to established clinical program protocols and timeframes to monitor their status, evaluate the effectiveness of the individualized plan of care, and identify new needs. Modify the individualized plan of care or case status based on the status of the member/patient.
- Contact members/patients to determine if there is a need for case management intervention based on protocols and timeframes established for the applicable clinical program.
- Conduct comprehensive assessments that include the medical, behavioral, pharmacy, and social needs of the member/patient. Review UPMC Health Plan and EMR data for services the member/patient has received and identify gaps in care based on clinical standards of care.
- Perform duties and responsibilities in accordance with the philosophy and standards of UPMC, including conveying courtesy, respect, enthusiasm, and a positive attitude through contacts with staff, members/patients, peers, and external contacts.
Qualifications
- Five years of experience in clinical, utilization management, home care, discharge planning, and/or case management required.
- BSN or MSN preferred.
- Three years of experience in a managed care environment preferred.
- Ability to interact with physicians and other health care professionals in a professional manner required.
- Computer proficiency required. Excellent verbal and written communication and interpersonal skills required
- Prior ED/EMS and case manager experience strongly preferred
