UPMC is looking for a full-time Home Care Aide for Family Hospice in Centre County. This Position is Monday-Friday with flexible scheduling, no on-call requirements, and requires some weekend rotations. This position provides educational and growth opportunities!
Responsibilities:
- Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
- Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
- Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
- Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
- Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
- Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
- Proven ability to facilitate change.
- Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
- Participates in all required agency meetings.
- Must maintain annual competency evaluation scores as determined by regulatory requirements

Qualifications
- High School Diploma or GED - Required
- One (1) year experience - Preferred
- Certified Nurse Aide -Preferred
- Minimum of 75 hours of Nurse Aide training- Required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.
Licensure, Certifications, and Clearances:
- Preferred: Nurse Aide Registry in good standing either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.
- Required: CPR is required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
