UPMC Health Plan is hiring a full-time Community Paramedic to support the Connect Community Paramedics department. This is a Monday through Friday daylight role and will be based out of the Pittsburgh (Northside - Harbor Gardens), PA office location. Travel within the Southwestern PA region will be required.
The Community Paramedic is responsible for staffing the daily operations of the CONNECT- Community Paramedic Program. They will be responsible for connecting with vulnerable patients and introducing them to available social determinants of health resources that could improve patients' lives while promoting healthy steps, disease management skills, care transitions, performing clinical assessments and vital signs, and other services that support operational efforts.
Responsibilities:
- Face-to-face management and follow-up on individual patient caseloads using a patient-centered approach.
- Complete and document patient biopsychosocial assessments.
- Connect patients to appropriate and various social service programs to reduce gaps in care.
- Conduct face-to-face, virtual, or telephonic patient interactions as appropriate.
- Professionally and timely document all patient interactions within the Health Plan's documentation system.
- Perform clinical assessments and vitals as directed and appropriate.
- Complete all training and competencies required within established timeframes.
- Interact with EMS personnel, hospital staff, and Care Management team members to coordinate patient care.
- Research and connect patients with community resources.
- Remain flexible to future workflow changes to support program growth.
Qualifications
- High school diploma or equivalent required.
- Current (in good standing) EMS licensure with minimum 1-year experience in public health, EMS, health screening, or another health-related field.
- Previous EMS licensure will be considered AND must be reinstated within one year of the hire date.
- Proficient in documentation platforms and computer/virtual platforms.
- Ability to interact professionally and effectively with patients, management, and other staff.
- Reliable transportation.
- Ability to work early morning hours and weekend hours as necessary.
- Ability to work a 40-hour week Monday - Friday.
Licensure, Certifications, and Clearances:
- Emergency Medical Technician (EMT), Advanced EMT, OR Paramedic
- Drivers' license required
- Automotive Insurance
- Act 31 Child Abuse Reporting with renewal
- Act 33 with renewal
- Act 34 with renewal
- Act 73 FBI Clearance with renewal
