Description
The Peer Navigator will perform a vital role on the multidisciplinary team by directly working with individuals to become self-advocates along their diagnosis and treatment journeys. The Peer Navigator will be pivotal in increasing consumer engagement across the department through the Consumer Advisory Board, Support Groups, and Clinical Quality Improvement projects. Additionally, this role will include performing rapid HIV testing, collection of STI tests, and linkage to HIV care, when indicated. Ample training and shadowing will be provided by the REACCH Program, the Pennsylvania Department of Health, and the program’s fiscal intermediary. This position will report to the REACCH Program’s Case Management and Social Work Supervisor. Supervision will be provided for a minimum of one hour per week.
Responsibilities:
Primary Duties – Peer support and client engagement:
- Serve clients and the REACCH program as a voice of experience by proactively discussing shared lived experiences and connecting to needed supports and services.
- Work one-on-one and in a group setting with clients to become self-advocates along their HIV diagnosis and treatment journeys.
- Maintain a caseload of approximately 25-30 clients in need of Peer Navigation services and develop a support plan with clients at least twice per year.
- Offer a safe space for clients to express their perceived strengths, barriers to treatment, and explore individualized strategies for success.
- Provide medication reminders and empowerment.
- Co-facilitate monthly REACCH Support group, provide clients with meeting reminders, coordinate day-of activities, etc.
- Co-facilitate quarterly Consumer Advisory Board (CAB) meetings, including provide clients with meeting reminders, coordinate day-of activities, help design marketing materials, obtain critical client feedback about programs and share back to supervisor and appropriate teams.
- Participate in quarterly clinical quality improvement committee meetings.
- Provide prompt follow-up with newly diagnosed clients at UPMC hospitals, off-site outpatient offices, and on-site at the REACCH Program.
Secondary Duties:
- Participate in community outreach events and education with REACCH team members.
- Provide rapid HIV and STI testing on-site at the REACCH Program and off-site in the community.
- Complete outreach to lost to care clients in emergency departments, client homes, and the community, as needed.
- Complete other tasks as assigned.
Qualifications
- High school diploma required, associate’s degree in related health or human services field preferred.
- Personal lived experience with HIV/AIDS preferred.
- Work experience in the HIV/AIDS field preferred.
- Demonstrated experience and ability to work with people with diverse and marginalized backgrounds, such as LGBTQ+ and/or BIPOC clients preferred.
- An ability to demonstrate one’s commitment to social justice preferred.
- Understanding of and the ability to apply motivational interviewing, solution-focused, and strengths-based client-focused techniques preferred.
