UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. The Jan. 13 Supreme Court of the United States decision that the Centers for Medicare & Medicaid Services federal COVID-19 vaccine mandate will move forward requires UPMC to ensure employees either get vaccinated or receive a requested medical or religious exemption.

GROWING OUR UPMC HOME HOSPICE TEAM

Are you a nurse looking for a rewarding career where you can provide direct, one-on-one patient care? UPMC Family Hospice is hiring a caring, skilled, and compassionate Home Care Aide to join our team Full-time! You’ll work in a home setting with a diverse patient population throughout the Tioga, and Potter County areas. Working outside of a medical facility, you’ll have an opportunity to get to know patients as you guide their care in the comfort of their homes.

Monday - Friday daylight shifts with rotations for weekends. 


Responsibilities:

  • Must maintain annual competency evaluation scores as determined by regulatory requirements
  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
  • Proven ability to facilitate change.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Participates in all required agency meetings.
  • Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient?s condition to appropriate professional staff.
  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
  • Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.

 

Qualifications

  • High School Diploma or GED - Required
  • One (1) year experience - Preferred
  • Certified Nurse Aide- Preferred
  • Minimum of 75 hours of Nurse Aide training Required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.


Licensure, Certifications, and Clearances:

  • Automotive Insurance
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Driver's License
  • Act 33 with renewal
  • Act 34
  • OAPSA
