UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. The Jan. 13 Supreme Court of the United States decision that the Centers for Medicare & Medicaid Services federal COVID-19 vaccine mandate will move forward requires UPMC to ensure employees either get vaccinated or receive a requested medical or religious exemption.

Discharge Specialist - UPMC McKeesport

UPMC McKeesport is currently looking for a fulltime Discharge Specialist to work a daylight shift (8 am – 4:30 pm), Monday thru Friday.

Purpose:
Work collaboratively with the social worker to facilitate discharge planning for patients hospitalized on the adult and aging inpatient units.

Responsibilities:

  • Communicate issues related to discharge, placement, and after care with social workers and other treatment team members.
  • Assist inpatient social workers in facilitating discharge, placement, mental health care and community resources for patients and families.
  • Assist inpatient social workers in completing necessary paperwork related to disposition planning.
  • Maintain collaborative relations with UPMC programs and with community agencies and organizations.
  • Assist in assuring appropriate releases of information are obtained for collaborative contacts.
  • Assist in copying and faxing appropriate clinical information related to continuity of care.

 

Qualifications

  • B.A. degree in a Mental Health Related field is required.
  • Knowledge of mental health and community resources preferred.

Licensure, Certifications, and Clearances:

  • Cardiopulmonary Resuscitation (CPR)
  • Comprehensive Crisis Management (CCMC) - Crisis Management training required within 60 days of hire
  • Act 33 with renewal
  • Act 34
