The Erie’s Public School Case Management program is dually based out of one of the Erie Public Schools buildings and UPMC Western Behavioral Health at Safe Harbor.
The Erie Public School Case Manager will receive referrals from the Erie School District and the Erie County community. Erie’s Public Schools Case Managers will provide support in the following domains: housing truancy/chronic absenteeism, mental health medical, substance abuse, legal, trauma, basic needs, cultural and language, employment, family dynamics, home behavior, activities of daily living, and more. Erie’s Public Schools Case Managers will provide case management services to remove barriers to learning, commit to working to reduce truancy and chronic absenteeism, increase family and community engagement collaborate with schools, Community School Directors, SAP teams, student services, and providers, community partnerships, and make referrals to increase social and mental health services and supports.
We are hiring for both Case Manager I and Case Manager II positions and offering a Sign On Bonus!

To qualify as a Case Manager I:
- Bachelor's Degree in Sociology, Social Work, Psychology, Criminology or related field required.
- Minimum of one-year related work experience is preferred.
- Access to transportation in the Agency service area with a valid Pennsylvania driver's license and auto liability insurance.
To qualify as a Case Manager II (starting rate $17.30):
- Bachelor's Degree in Sociology, Social Work, Psychology, Criminology or related field required. Master's Degree in Sociology, Social Work, Psychology, Criminology, or related field preferred.
- Minimum of three years of related work experience is required.
- Access to transportation in the Agency service area with a valid Pennsylvania driver's license and auto liability insurance.
Responsibilities:
- Will provide case management services, using a system of recovery that integrates mental health services, drug and alcohol services, and community resources, to an assigned caseload. Identify the psychiatric, social, emotional, educational, housing, vocational and economic needs of individuals through a continuous process in conjunction with the individual, involved support members and supervisor. The emphasis during assessment shall be placed on assisting the individual in maximizing their success in the community through successful use of the lowest levels of care possible, formal and informal support, and in accordance with the individual's strengths, interests, and goals.
- As applicable, staff will: Complete assessments for all individuals to determine case management needs and activities Develop an agreed-upon plan for case management services, which may include a collaborative recovery-oriented service plan in cooperation with the individual, supervisor, and other professionals.
- Assist in writing Safety Crisis Plans. Serve in the capacity of liaison either linked to or embedded within clinics, hospitals, or other community partners as assigned and provide requested case management services to individuals served by those entities.
- Provide case management services that will assist individuals in attaining the following outcomes: Increasing community living through diversion from hospitalization, and incarceration, and building community resources. Engaging in mental health treatment. Facilitate crisis intervention services according to program guidelines. Entrance into appropriate vocational or educational services and activities. Implement service plans in cooperation with the individual, their support and other professionals.
- Provide various behavioral health training to staff at Personal Care Home settings in order to enhance their understanding of and professional competence in working with our shared clients and any clients they house that are living with psychiatric disability. Serve in the capacity of liaison either linked to or embedded within clinics, hospitals, or other community partners as assigned and provide requested case management services to individuals served by those entities.
- Evaluate the individual's progress based upon the agreed case management services and/or service plans at required times and as special needs arise. Revise case management activities and/or service plans in cooperation with the individual, their supports, other team members, and supervisor. Refer individuals to allied community organizations and agencies for assistance with needs that cannot be met by the program or Agency. Advocate for individuals in securing necessary services and resources to meet their needs. Communicate effectively regarding individual progress through reports, records, and team conferences. Assist individuals and families managing personal crisis.
- Provide case management services which will assist individuals in attaining the following outcomes: Increasing community living through diversion from hospitalization, incarceration, and building community resources. Engaging in mental health treatment. Facilitate crisis intervention services according to program guidelines. Entrance into appropriate vocational or educational services and activities.
- Function in the Case Manager role utilizing work time under the following guidelines: Maintain contact and services to clients according to program standards and description. Complete all paperwork required by regulations and program standards. Participate in on-call rotation as applicable to the program.
- Assist supervisor with creating call schedule, running team meetings, or other administrative support.
- Serve as peer trainer and/or mentor for new case management staff.
- Represent the case management program in internal and external treatment team meetings a proxy for the team supervisor(s).
- Lead program outreach activities as assigned.
- Participate in educational programs, quality assurance, and quality improvement activities.
- Maintain and protect the individual and Agency confidentiality.
- Promote personal and co-worker safety during work duties.
- Represent and participate in Agency professional and community activities as requested.
- Endeavor to achieve and maintain quality and consumer satisfaction in the delivery of services to internal/external customers.
- Demonstrate support of Agency mission and philosophy in delivery of services.
- Demonstrates competency in welcoming expectations.
