UPMC Altoona is seeking a Registered Nurse - Healthy Beginnings Care Coordinator to support our Pregnancy Care Center! This role will work Monday - Friday during daylight hours. We are looking for a candidate with 5 years of OB experience and office experience!
Purpose:
Coordination and provision of prenatal care to low-income pregnant women.
Responsibilities:
- Planning
- a. Includes patient/family in the information of a plan of care for each assigned patient.
- b. Prioritizes patient's needs
- c. Modifies plan if care in response to changing patient needs.
- d. Utilizes appropriate community resources in planning care.
- Coordination
- a. Responsible for coordinating all aspects of patient care.
- b. Utilizes appropriate community agencies and other health care professionals in the provision of care.
- c. Coordinates information and referrals with primary care providers and other health care providers to assure continuity of care.
- Assessment
- a. Completes the collection of data for the assessment of each patient during the admission interview process according to the unit policies and standards.
- b. Reassesses patient each visit as per unit policies and standards.
- c. Documents data using appropriate forms and format.
- d. Recognizes normal physiology and pathophysiology of pregnancy and other conditions.
- e. Interprets diagnostic/monitoring data and recognizes significance based upon patient's condition.
- Implementation
- a. Implements interventions within the scope of nursing practice and established unit standards and protocols for care.
- b. Demonstrates ability to adapt nursing care and health education to all stages of the life cycle and to various stages of health and disease.
- c. Carries out and documents procedures according to hospital policy.
- d. Demonstrates knowledge of the physiological, psychosocial, emotional, and cultural basis for the delivery of nursing care.
- e. Provides health teaching for patients and families based on assessed needs and learning abilities of patient/family.
- f. Follows protocol for patient telephone advice and communication of test results to patients.
- g. Demonstrates knowledge of legal, ethical, and moral boundaries of nursing care delivery.
- Evaluation
- a. Evaluates and document the patient's response to care, teaching, and treatments.
- b. Evaluates patient's outcomes based on previously defined goals.
Qualifications
- Must be a graduate from an accredited school of nursing.
- Must hold a current license to practice nursing in the stats of PA.
- Must satisfactorily complete the hospitals and unit's orientation program and probationary period.
- At least 5 years experience in obstetrics.
- Maternal health education.
- Outpatient care or office experience.
Licensure, Certifications, and Clearances:
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
- Registered Nurse (RN)
- Act 33 with renewal
- Act 34 with renewal
- Act 73 FBI Clearance with renewal
