Community HealthChoices (CHC) is Pennsylvania's managed care long term services and supports (LTSS) program serving seniors and individuals with physical disabilities in the Commonwealth who are covered by Medicare and Medicaid. The goal of UPMC's CHC Housing Program is to assist our Participants as they transition from nursing home facilities to a community setting as well as to ensure stable housing for our participants living in the community. The Manager, Housing Strategy will provide the resources and oversite necessary to facilitate a seamless nursing home transition in conjunction with our service coordination team and community partners.

  • Counsel and assist NHT staff, NF SC staff, HCBS staff, and nursing home staff when necessary with informational and procedural matters
  • Develop and maintain policies and procedures on housing, which may include: participant eligibility, lease agreements, petitions, retention, emergency or temporary housing, special interest housing, and riders to leases
  • Develop and maintain policies and procedures to identify Participants' needs with nursing home and nursing home transition staff
  • Develop, implement, modify, and/or supervise programs and services in support of UPMC's CHC Housing Program
  • Duties for assigned off-nursing home site may include: serving as liaison with local landlords and dealing with landlord/tenant issues; developing and administering a mediation program, with responsibility for all related matters and staff; providing assistance and support in serving needs of commuter students
  • Consult and act as liaison with federal, state, and housing agencies to develop appropriate support and referral systems
  • Generate statistical reports and maintain records as required
  • Maintain and update daily computer-based filing system for nursing home transition staff assignments and billing; meeting with and counseling nursing home transition staff, as required, to resolve problems regarding housing assignments and approve and coordinate all changes
  • Maintain an inventory of housing related resources (i.e. apartment vacancies) and programs
  • Research on all appropriate program/service activities related to housing services (i.e. HUD Section 8 waiting lists, public housing stock)


  • Bachelors degree with a minimum of five years related housing or community development experience
  • Masters degree preferred
  • Knowledge of regulations impacting housing for target population
  • Proven human relations and counseling experience Strong organizational and supervisory skills
  • Excellent Oral and written communications skills
  • Computer skills: Microsoft: Excel, Outlook, PowerPoint, and Word
  • Experience with nursing home facility management and/or clinical expertise preferred
