UPMC Susquehanna is currently seeking a Supervisor of Clinical Services for the Home Health & Hospice office at the Wellsboro location. This is an excellent opportunity to utilize one's nursing and leadership skills to support a team of home health and hospital professionals provide care to the surrounding area.

Supervises frontline staff in Home Health and Hospice, assuming responsibility for their work performance and providing oversight of work flow and processes. Responsible for all clinical activities of assigned nursing and other professional staff. Oversees all case management activities and meets or exceeds established outcome measures.

Assumes responsibility for the timely completion of all legal and regulatory requirements of direct reports
  • Participates in and welcomes self-assessment opportunities. Participates in SH offered education activities and is fully engaged while in attendance.   • Supervises frontline staff in Home Health, assuming responsibility for their work performance and providing oversight of work flow and processes. Responsible for all clinical activities of assigned nursing and other professional staff.
  • Rewards, disciplines and evaluates direct reports, using independent judgment; provides input into hiring and involuntary termination decisions.
  • Provide input into department�s budget; supports the financial goals of Susquehanna Health through individual actions and management of resources.
  • Establish evidence-based protocols to guide effective practice.
  • Work directly with staff on patient experience, quality and cost measures to meet or exceed established targets.
  • Complete field visits at least monthly to assess care, service and patient satisfaction.
  • Evaluate patient care and outcomes through effective utilization of reports.
  • Coordinate education for staff, patients and families.
  • Review supply chain management protocols daily.
  • Ensure all productivity and documentation standards are met.


  • Current UPMC Susquehanna employees must obtain BSN within three years of hire. 
  • External candidates are required to have a Bachelors degree in Nursing upon hire. 
  • Two years of home health experience or two years of supervisory experience in a healthcare setting required.

Licensure, Certifications, and Clearances:
Registered Nurse licensure in the Commonwealth of Pennsylvania Required AHA BLS for Healthcare Providers required within 90 days of hire
