Are you seeking a rewarding pharmacy career? This role will coordinate clinical pharmacy services and ensures the pharmacy provides optimal clinical services on a daily basis for patients in post-acute care areas of practice. Facilitate the development, implementation, and maintenance of systems that promote desirable patient outcomes through the use of safe, cost-effective drug therapies.

  • Demonstrates required attendance and punctuality.
  • Document activities through departmental reports, software and quality assurance mechanisms.
  • Interact routinely with physicians and nurses to provide optimal pharmaceutical care to patients.
  • Maintain expertise in area(s) of practice through continual learning experience.
  • Monitor and instruct technical staff for correctness of order entry, medication delivery, and overall operations.
  • Perform DUR review of all orders.
  • Perform pharmacotherapeutic monitoring of patients in area of service responsibility in a routine, continuous manner taking into account the age and clinical condition of each patient.
  • Perform prospective drug review of all new orders and make recommendations for alternative medications where applicable.
  • Provide and document pharmacotherapeutic recommendations that positively affect patient outcome.
  • Provide general and patient-specific drug information to patients, physicians, nurses and other healthcare professionals.
  • Support other staff pharmacists in providing pharmaceutical care to patients.
  • Support pharmacy-related research.
  • Works with pharmacy staff to effectively cover the hours of operations and on call hours.


  • B.S. degree from an accredited School of Pharmacy & 10 years of relevant Pharmacy experience or PharmD required. 
  • Ability to obtain and interpret information related to the drug treatment processes and best practices. 
  • Possess strong interpersonal, verbal and written communication skills. 
  • Understands clinical pharmacy practices and activities. 
  • Clinical chart review experience preferred. .

Licensure, Certifications, and Clearances:
  • Act 34 Criminal Clearance
  • Pharmacist
