Join the UPMC Home Healthcare Palliative Care Team as the Team Leader. Manage and supervise field nurses providing palliation to patients in the Greater Pittsburgh Area. Effect quality patient care through accurate documentation, ongoing education and coordination of team operations.


  • Oversee team activities including assignment of new patients to multidisciplinary staff, order verification and coordination of visits to maximize productivity
  • Provide discipline specific direct patient care as scheduling requires.
  • Assume responsibility for ongoing team education and facilitate accurate documentation. Perform supervisory visits with staff to ensure delivery of quality care
  • Collaborate with clinical supervisor and team assistant
  • Adhere to organizational standards, policies and procedures. Ensure optimal patient care and outcomes while containing cost per episode
  • Identify, develop and participate in process improvements to enhance quality service and facilitate change
  • Foster and promote positive work environment and communicate effectively with team members, patients and caregivers, staff, physicians and other professionals
  • Focus on customer service by performing duties in a manner resulting in optimal patient satisfaction
  • The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below. They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures
  • Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families
  • Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment
  • Exemplary clinical judgment with critical thinking, analytical and problem solving abilities required as related to various aspects of patient care
  • Critical thinking skills necessary to exercise and to lead others in application of the nursing process


  • Palliative experience preferred
  • Oncology experience preferred
  • IV Skills preferred
  • Demonstrated competence as a Professional Staff Nurse
  • Baccalaureate degree-Preferred
  • Minimum of two (2) years healthcare experience-Required
  • Minimum of one (1) year home health experience-Required
  • Minimum of one (1) year management experience-Preferred

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Act 33 Child Clearance
  • Act 34 Criminal Clearance
  • Automotive Insurance
  • Basic Life Support or Cardio Pulmonary Resuscitation
  • Driver's License
  • Older Adult Protective Services Act
  • Registered Nurse
