Purpose:

The Pharmacist ensures optimal patient outcomes; educates present and future practitioners and supporting drug-related research. These services will be provided in compliance with state and federal regulations, JCAHO standards, the mission, policies and procedures of the hospital and department. Pharmacist will be involved in patient care activities supporting a variety of patient populations ranging from adolescents to geriatrics. Inpatient and outpatient practice areas include, but are not limited to, general medicine, surgery, intensive care, psychiatry, orthopedics, oncology and cardiology.

Responsibilities:
  • Actively participates in the professional growth and development of new employees, students, and resident(s).
  • Adhere to all regulatory requirements, professional standards of practice, and hospital quality management policies and procedures in providing pharmaceutical care.
  • Annually complete continuing education programs related to Pharmacy Practice as a mandatory requirement for maintaining an active license in the Commonwealth of Pennsylvania.
  • Audit and maintain accurate records for all controlled substances. Assure that all controlled substances are stored and dispensed in compliance with Federal, State, and Departmental regulations.
  • Communicate with physicians and nurses to insure prompt clarification of orders that are unclear, illegible, or incomplete.
  • Demonstrate behaviors that support the Mission, Vision, and Values Statement of the hospital, the Department of Pharmacy and Therapeutics and contribute to the overall success of our organization.
  • Fill prescription orders in accordance with State and Federal regulations, utilizing technological resources and equipment.
  • Interpret and evaluate physician orders in order to prepare, calculate, and dispense appropriate medication dosage forms for inpatient and outpatient treatment in accordance with the Pennsylvania Practice Act, Federal Drug Laws and other regulatory organizations.
  • Organize daily operational duties and assist in the supervision of pharmacy technicians, pharmacy aides, student interns and other ancillary personnel involved in providing pharmaceutical care.
  • Participates in the training of new employees.
  • Prepare and dispense intravenous admixture solutions, as required, using aseptic technique in an accepted sterile product preparation area.
  • Provide drug information and/or educational programs to healthcare professionals and pharmacy students.
  • Responds to arrest codes and participates as required.
  • Review medication orders for safe and effective indication, dose, route, and scheduled administration. This monitoring and evaluation is appropriately documented.
  • Review, maintain, and screen patient medication records and profiles for allergies, drug interactions and therapeutic duplications while entering orders and/or verifying medications.
  • Screen all medication orders for Adverse Drug Reactions/Medication Errors as per Department standards and report through appropriate reporting process.
  • Support Drug Use Disease State Management initiatives conducted by the Department.
  • Support drug-related research by assisting in the procurement, accountability, storage, preparation, and distribution of investigational/study drugs.
  • Sustain the formulary by minimizing non-formulary procurements, utilizing therapeutic interchange protocols, and promoting rational drug therapy selection.
  • Upon becoming aware of a dispensed medication variance, ensures patient safety by notifying patient's physician and nurse in a timely fashion. If possible, immediately retrieves, or oversees retrieval of, incorrect product. If necessary, prepares and dispenses, or oversees preparation and dispensing of, correct product.
  • Utilize all available technologies in the provision of pharmaceutical care (i.e., computer systems and databases, Acu-Dose, Robot-RX, pneumatic tube system, etc).
  • Where applicable, assist in the completion of monthly Nursing Unit liaison reports/emergency medication cart checks by the last day of each month following Department and JCAHO Guidelines.
  • Where applicable, document clinical interventions in accordance with Department procedures.

Qualifications

  • Bachelors degree in Pharmacy or Pharmacy degree from an accredited School of Pharmacy. 
  • Good communication skills. 
  • Post-licensure hospital pharmacy experience preferred.
  • Pediatric pharmacy experience is preferred.

Licensure, Certifications, and Clearances:
  • Act 33 Child Clearance with Renewal
  • Act 34 Criminal Clearance with Renewal
  • Act 73 FBI Clearance
  • Pharmacist
